It contained an emergency <br />coordinator and phone number of past managers, who presently does not work there. There must be at least one <br />emergency coordinator on site or on call to coordinate emergency response measures, and the following <br />information must be posted by a phone: the name and phone number of the emergency coordinator; location of fire <br />extinguishers, spill control equipment, and if present, fire alarm; and the phone number of the fire department, <br />unless the facility has a direct alarm. Immediately appoint an emergency coordinator and post the required <br />information by a phone. A form is provided that can be used for this purpose. <br />This was corrected on site. <br />This is a repeat violation, Class II. <br />109 <br />CCR 66262.23(a)(4) Failed to send generator manifest copies to DTSC within 30 days. <br />Generator copies of manifest number 009561901 FILE were not sent to DTSC within 30 days of shipment of <br />hazardous waste. The generator manifest copy shall be submitted to DTSC for every shipment on a manifest when <br />California is either the generator state or the destination or consignment state. The generator manifest copy shall <br />be mailed to: <br />DTSC Generator Manifests <br />P.O. Box 400 <br />Sacramento, CA 95812-0400 <br />Immediately mail a copy of the manifests listed and ensure that copies of all generator manifests are sent to DTSC <br />within 30 days of shipping hazardous waste. <br />This is a Class II violation. <br />Overall Inspection Comments: <br />Onsite for routine hazardous waste inspection. According to Matt Duce (Manager), a mobile truck care shop had <br />recently opened, and can potentially affect their generator status. <br />Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br />documenting the corrective actions that have been or will be taken for each violation, and any supporting <br />paperwork, within 30 days of receipt of this report. <br />Please be aware as of January 1, 2013, all businesses are required to submit all hazardous materials <br />information online to the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov.
