Financial responsibility documents submitted to the EHD a copy was not available on site during inspection. The financial responsibility documents on site are dated 01/15/2018 and the chief financial officer letter dated 4/30/2018. Current financial responsibility documents are required to be submitted annually and maintain a copy on site. Immediately locate the required documents and ensure a copy is available on site for inspection at any time. This is a minor violation. 118 CCR 2715(a)(1)(A), 2715(a)(2) Failure to submit the "Underground Storage Tank Statement of Understanding and Compliance Form". The Underground Storage Tank Statement of Understanding and Compliance Form was not found in CERS. This form must be uploaded to the California Environmental Reporting System (CERS). Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov and upload the Underground Storage Tank Statement of Understanding and Compliance Form for this facility This is a minor violation. 202 CCR 2715(c)(3), 2715(c)(4) Failure to maintain the "Facility Employee Training Certificate" on-site (after October 1, 2018). The Facility Employee Training Certificate dated 10/30/18 contained no"initial training dates" and the "date of assuming responsibility as a facility employee"for listed employees (Wagma Abdul, Hasina Nasir and Navjot). All site employees must be trained prior to taking responsibility as a facility employee if after October 1, 2018, or within 30 days of hire if prior to October 1, 2018. The Facility Employee Training Certificate shall contain accurate information for all employees trained. Update the Facility Employee Training Certificate to contain the accurate information and submit a copy of the updated form to the EHD. This is a minor violation. 320 CCR 2635, 2637.2, 2665, 2712 Failure to comply with the installation, inspection, or record keeping of the overfill prevention. Overfill prevention testing has not been conducted. This testing was required to be conducted by October 13, 2018. Immediately schedule this testing and provide 48 hours notification to the EHD. Ensure all future testing is conducted within required time frames. This is a Class II violation.
