The response plan is not current and/or not approved by the EHD. The response plan must be uploaded to the California Environmental Reporting System (CERS). Immediately log into CERS, upload the completed form, and submit for review by the EHD. Ensure a copy is available for inspection at any time. This is a minor violation. 204 CCR 2712(i)Current monitoring plan approved by the EHD not found on site. An approved copy of the monitoring plans were not available for inspection. A copy of these plans shall be accessible on site at all times. Immediately locate and retain a copy on site. This is a minor violation. 205 CCR 2712(i), 2634(e)Failure to submit a current UST Response Plan and have it available onsite. The response plan is not current and/or not approved by the EHD. Store manager, Hassan Ahmed is noted to be one of the responsible person in the response plan, however, according to Nofal Mohsin, Hassan Ahmed is no longer working at the facility.The response plan must be updated and uploaded to the California Environmental Reporting System (CERS). Immediately log into CERS, upload the completed form, and submit for review by the EHD. Ensure a copy is available for inspection at any time. This is a minor violation. 209 CCR 2716(a-e) Failure to conduct the designated UST operator visual inspection at least once every 30 days. 1) A designated operator inspection was performed on November 15, 2018 and the next inspection was performed on December 17, 2018, 32 days later. The designated operator visual inspections shall be conducted within 30 days or less of the previous inspection. Ensure that all designated operator inspections are performed within the required time frames. 2)The designated operator failed to check and note all required items on the visual inspection reports. -Section V of the inspection reports were missing follow up actions about scheduling Overfill Prevention Equipment Testing. -Section VI of the inspection reports were missing the date, name and signature of the owner/operator. During the visual inspection,the designated operator shall check that all required testing and maintenance for the UST system have been completed and shall check all required items on the report. Ensure that designated operators performing visual inspections at this facility are including all of the required information on the reports. This is a minor violation.
