The designated operator visual inspections shall be conducted within 30 days or less <br /> of the previous inspection. Ensure that all designated operator inspections are performed within the required time <br /> frames. <br /> 2)The designated operator failed to check and note all required items on the visual inspection reports. <br /> -Tank and line tightness test information was left blank. <br /> -Missing UST/Operator signature. <br /> -Overfill Prevention testing was incorrectly stated as 1/16/2019. <br /> During the visual inspection,the designated operator shall check all required testing and maintenance for the UST <br /> system have been completed and shall check all required items on the report. Ensure that designed operators <br /> performing visual inspections at this facility are including all of the required information on the reports. <br /> This is a repeat violation, Class Il. <br /> 306 CCR 2630(d), 2641(a) Monitoring equipment is not installed or maintained to detect a leak at the earliest <br /> opportunity. <br /> OBSERVATION: The ULS sensors in UDC's 1/2 and 3/4 were observed on their side(approximately less than 45 <br /> degrees)from the lowest point of the sump and not located to detect a leak at the earliest opportunity. <br /> REGULATION GUIDANCE: Monitoring equipment shall be maintained to be able to detect a leak at the earliest <br /> possible opportunity. <br /> CORRECTIVE ACTION: The sensor was relocated to the lowest point of the sump by the service technician. <br /> Ensure that all monitoring equipment is maintained and located to detect a leak at the earliest opportunity. <br /> This was corrected on site. <br /> This is a repeat violation, Class Il. <br /> 313 HSC 25290.1,25290.2, 25291 Failure to construct, operate, and maintain primary containment as product-tight. <br /> OBSERVATION: Observed a small amount of liquid in the 1/2, 3/4, and 7/8 UDC sumps, indicating a leak in the <br /> primary containment. <br /> REGULATION GUIDANCE:All primary containment for the UST system must be product tight. <br /> CORRECTIVE ACTION: Immediately have a properly licensed,trained, and certified contractor repair or replace the <br /> failed component(s), under permit and inspection of the EHD if necessary. <br /> This was corrected on site. <br /> This is a Class II violation. <br /> FA0000279 PR0231497 SCO01 12/02/2019 <br /> EHD 23-01 Rev.9/20/2019 Page 5 of 6 Underground Storage Tank Program OIR <br /> 1868 E.
