Contact the local fire department and the San Joaquin Valley Air Pollution Control <br /> District for any applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval <br /> prior to commencing work. All new addenda supersede the previous ones. <br /> 3. Contact Paul Nso at (209) 468-3432 to arrange the following inspection appointments <br /> (a 48-hour notice is required for inspection appointments). <br /> a. Removal of tank and any applicable piping <br /> b. Verification that tank is cleaned to Title 22 CCR 67383.3(e)(2) standards <br /> c. Witness soil and/or water sampling <br /> d. Verification of Tank ID number marked on the UST <br /> 4. If the removed backfill material and soil is found to be contaminated it must be <br /> disposed of at an approved disposal location. Treatment of soil onsite can only be <br /> performed under a hazardous waste treatment permit. <br /> 5. Inert UST with 30 pounds of dry ice per 1,000 gallon volume after triple rinsing. Be <br /> aware, additional testing/monitoring to ensure tanks remain inerted may be required by <br /> the local Fire Department. <br /> 6. A copy of the submitted "Health and Safety Plan" must be on the jobsite for review. <br /> 1868 E.
