The Facility Employee Training Certificate dated 1/10/19 listed the employee Valeria Garcia to have"initial training date"of 1/10/19 which was after the"date of assuming responsibility as a facility employee"of 11/10/18. Furthermore the site manager's"date of assuming responsibility as a facility employee"was not listed, however the manager was able to add the missing date to correct this part of the violation while the inspector was on site. All site employees must be trained prior to taking responsibility as a facility employee if after October 1, 2018, or within 30 days of hire if prior to October 1, 2018. The Facility Employee Training Certificate shall contain accurate information for all employees trained. Ensure that all site employees are trained prior to taking responsibility as a facility employee. This is a repeat violation, Class II. 204 CCR 2712(i)Current monitoring plan approved by the EHD not found on site. An approved copy of the monitoring plans were not available for inspection. A copy of these plans shall be accessible on site at all times. The manager Chris Arbabian was able to get the monitoring plan emailed to him to correct this while the inspector was on site.
