There are violations that are still <br /> outstanding : <br /> Violations that remain open : <br /> -Failure to maintain the " Facility Employee Training Certificate " on- site (after October 1 , 2018 ) <br /> -Monitoring in sump / UDC failed to activate an audible and visual alarm or stop the flow product. <br /> -Failure to construct, operate , and maintain primary containment as product-tight . <br /> -Leak detector failed to detect a 3 . 0 gallons per hour leak at 10 pounds per square inch . <br /> -Failure to comply with the installation, inspection, or record keeping of the overfill <br /> Prevention . <br /> I reviewed the work order provided in the email and was not able to close any of the violations because the <br /> work done did not address any of the issues . The work order states that parts are needed to make repairs . Please <br /> ensure that the outstanding violations are addresses .
