A CERS submittal uploaded on 1/3/19 was made with the"Designated Underground Storage Tank Operator Identification Form,"and an"Underground Storage Tank Statement of Understanding and Compliance Form,"was not uploaded. 'The owner or operator shall submit a signed"Underground Storage Tank Statement of Understanding and Compliance Form,"upon any change in the owner or operator which previously submitted the signed "Underground Storage Tank Statement of Understanding and Compliance Form," no later than 30 days after the change. Since a"Designated Underground Storage Tank Operator Identification Form,"has been uploaded after October 1st,2018. It is required for a singed"Underground Storage Tank Statement of Understanding and Compliance Form,"to be uploaded.Any changes shall be reported to the EHD within 30 days. Immediately log into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov and upload a signed "Underground Storage Tank Statement of Understanding and Compliance Form"for this facility. This is a minor violation. 201 CCR 2712(b)(1-2)Failure to maintain monitoring, maintenance, and follow up action records. Maintenance and monitoring records for the last three years were not found on site. Retest for Secondary containment was conducted on 8/13/18 under SR0079240 and those records were not found on site. These records shall be maintained on site for at least three years. Monitoring records include: (1)date and time of all monitoring or sampling; (2)monitoring equipment calibration and maintenance records; (3)results of any visual observations; (4) results of sample analysis performed a lab or in the field; (5)logs of all readings of gauges or other monitoring equipment,ground water elevations, or other test results; (6)results of inventory readings and reconciliations. Immediately locate and maintain all maintenance and monitoring records for the last three years on site and submit copies to the EHD. This is a repeat violation, Class II. 202 CCR 2715(c)(3), 2715(c)(4) Failure to maintain the"Facility Employee Training Certificate"on-site(after October 1, 2018). Facility Employee Training Certificate"dated 4/29/19 was found on site missing CERS ID All site employees must be trained prior to taking responsibility as a facility employee if after October 1, 2018, or within 30 days of hire if prior to October 1,2018.The"Facility Employee Training Certificate"shall contain accurate information for all employees trained. Update the"Facility Employee Training Certificate"to contain the accurate information and submit a copy of the updated form to the EHD. This is a minor violation.
