SAN <br /> J D �� I Environmental Health Department <br /> COUNTY— <br /> Underground Storage Tank Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> QUIK STOP MARKET#2076* 1030 S OLIVE ST, STOCKTON July 01, 2019 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 209 CCR 2716(a-e) Failure to conduct the designated UST operator visual inspection at least once every 30 days. <br /> The designated operator failed to check and note all required items on the visual inspection reports. <br /> The following was observed on the designated operator's monthly inspection reports: <br /> -Section I was missing CERS ID. <br /> -Section III of the report was left blank. <br /> -The"NA"in sections VII through XI were not explained. <br /> -Section VI of the report was missing date, name and signature of the owner/operator. <br /> -Overfill prevention testing date was incorrectly stated to be 10/1/2018.According to test results seen onsite,this <br /> test was completed on 9/25/2018. <br /> -The designated operator documented visual inspections of sumps and UDCs, but left blank the question regarding <br /> sump and UDC sensor locations. <br /> During the visual inspection,the designated operator shall check all required testing and maintenance for the UST <br /> system have been completed and shall check all required items on the report. Ensure that designed operators <br /> performing visual inspections at this facility are including all of the required information on the reports. <br /> This is a minor violation. <br /> 315 HSC 25291(e)Water in secondary containment not removed, analyzed, and properly disposed of(pre-Jul 2003). <br /> At the time of the inspection, the following amount of liquid was observed: <br /> -More than 30 gallons of liquid was observed in the 87 product fill sump. <br /> -Approximately 20 gallons of liquid was observed in the 91 product fill sump. <br /> If water could enter into the secondary containment by precipitation or infiltration, it must be removed and disposed <br /> of properly.While the inspector was on site,the service technicians started the removal of all liquid from 87 product <br /> and 91 product fill sumps. Ensure that all sumps and annular spaces are maintained free of liquid. <br /> This was corrected on site. <br /> This is a minor violation. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by July 31, 2019. <br /> Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate ($152).
