The incorrect and <br /> missing information included the following from the inspection reports: <br /> -Section V was left blank. <br /> -CERS ID was left blank. <br /> -Overfill test information was left blank. <br /> -The reports incorrectly mark that all follow up to previously reported issues were completed and list repeat issues in <br /> Section III. <br /> -Overfill test date was left blank. <br /> -Section VI was left blank. <br /> During the visual inspection, the designated operator shall check that all required testing and maintenance for the UST <br /> system have been completed and shall check all required items on the report. Ensure that designated operators <br /> performing visual inspections at this facility are including all of the required information on the reports. <br /> This is a Class II violation. <br /> 215 CCR 2712(1) Failure to retain a copy of the permit to operate at the facility. <br /> A copy of the UST operating permit was not found on site. A copy of the UST operating permit and all attachments <br /> and conditions shall be retained at the facility. Immediately obtain a copy of the permit and retain it on site. <br /> This is a Class II violation. <br /> 314 HSC 25290.1, 25290.2, 25291 Failed to have secondary containment or secondary containment not tight. <br /> The S3 (91 line)sensor has been in no vacuum alarm since May 6, 2019. Secondary containment shall be impervious <br /> to the liquid and vapor of the substance contained and constructed to prevent structural weakening as a result of <br /> contact with any hazardous substances released from the primary containment. Immediately contact a properly <br /> licensed, trained, and certified contractor to make repairs to the UST system under permit and inspection of the EHD. <br /> A permit application (SR0080525) has been received by EHD for repairing a damaged product pipe, but has not yet <br /> been accepted. <br /> This is a Class II violation. <br /> Overall Inspection Comments: <br /> Witnessed testing of the 87 and 91 spill containers on April 30, 2019. Both passed. Verified 5 gallon capacity. <br /> This testing completed the annual monitoring inspection which was due by April 2019. <br /> Sensor functionality, VPH communication, and leak detector testing were conducted on March 25, 2019 under <br /> SR0078796; that testing will serve as the initial portion of the annual inspection, resetting the inspection due date <br /> to March 2020. <br /> The paperwork inspection was completed on May 30, 2019. <br /> An inspection checklist was provided to the facility operator on the day of inspection.
