The designated operator failed to check and note all required items on the visual inspection reports. The incorrect and missing information included the following from the inspection reports: -Section V was left blank. -CERS ID was left blank. -Overfill test information was left blank. -The reports incorrectly mark that all follow up to previously reported issues were completed and list repeat issues in Section III. -Overfill test date was left blank. -Section VI was left blank. During the visual inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and shall check all required items on the report. Ensure that designated operators performing visual inspections at this facility are including all of the required information on the reports. This is a Class II violation. 215 CCR 2712(1) Failure to retain a copy of the permit to operate at the facility. A copy of the UST operating permit was not found on site. A copy of the UST operating permit and all attachments and conditions shall be retained at the facility. Immediately obtain a copy of the permit and retain it on site. This is a Class II violation. 314 HSC 25290.1, 25290.2, 25291 Failed to have secondary containment or secondary containment not tight. The S3 (91 line)sensor has been in no vacuum alarm since May 6, 2019. Secondary containment shall be impervious to the liquid and vapor of the substance contained and constructed to prevent structural weakening as a result of contact with any hazardous substances released from the primary containment. Immediately contact a properly licensed, trained, and certified contractor to make repairs to the UST system under permit and inspection of the EHD. A permit application (SR0080525) has been received by EHD for repairing a damaged product pipe, but has not yet been accepted. This is a Class II violation. Overall Inspection Comments: Witnessed testing of the 87 and 91 spill containers on April 30, 2019. Both passed. Verified 5 gallon capacity. This testing completed the annual monitoring inspection which was due by April 2019. Sensor functionality, VPH communication, and leak detector testing were conducted on March 25, 2019 under SR0078796; that testing will serve as the initial portion of the annual inspection, resetting the inspection due date to March 2020. The paperwork inspection was completed on May 30, 2019. An inspection checklist was provided to the facility operator on the day of inspection.
