Overfill prevention testing was performed on 10/4/18 and a test report was not submitted to EHD. A copy of the test report must be submitted within 30 days of the tests. Take all necessary precautions to ensure testing and submittal of test reports are performed in a timely manner. This is a minor violation. 202 CCR 2715(c)(3), 2715(c)(4) Failure to maintain the "Facility Employee Training Certificate" on-site (after October 1, 2018). The designated operator employee training certificate conducted on 03/18/19. The CERS ID was left blank. The designated operator shall check that all required information is completed and shall check all required items on the report. Ensure that designated operators performing training at this facility shall provide all of the required information on the training certificate report. This is a minor violation. 209 CCR 2716(a-e) Failure to conduct the designated UST operator visual inspection at least once every 30 days. The designated operator failed to check and note all required items on the visual inspection reports. -The 3/18/19 inspection report included incorrect secondary containment test date (4/30/18). The correct date was 9/29/16. The CERS ID and section VI was left blank. -The 01/21/2019 inspection report left blank the CERS ID and section VI. The report incorrectly state the overfill test date (04/30/2018).The correct date was 10/4/2018. -The 10/01/18, 10/30/18 and 06/03/2019 inspection reports left blank the CERS ID and section VI. -The 11/27/18, and 12/26/18 inspection reports left blank the CERS ID, section V and VI. -The 02/19/2019 inspection report left blank the CERS ID and section VI. -The inspection reports of 04/8/2019 and 05/6/2019 was missing the CERS ID. -The inspection report of 07/01/2019 left blank the CERS ID, section V and VI. During the visual inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and shall check all required items on the report. Ensure that designated operators performing visual inspections at this facility are including all of the required information on the reports. This is a minor violation.
