The designated operator failed to check and note all required items on the visual inspection reports. -Designated operator's monthly inspection reports dated 10/10/2018 were completed on the old form. -Tank and line tightness test information was left blank on the November-July monthly inspection reports for all tanks. During the visual inspection,the designated operator shall check all required testing and maintenance for the UST system have been completed and shall check all required items on the report. Ensure that designed operators performing visual inspections at this facility are including all of the required information on the reports. This is a minor violation. 309 CCR 2636(f)(1)Monitoring in sump/UDC failed to activate an audible and visual alarm or stop the flow product. The Bravo Float and Chain in UDC 1, 2, and the diesel dispenser failed to stop the flow of product when tested to ensure continuous monitoring of the tank system.All monitoring equipment shall be maintained to activate an audible and visual alarm or stop the flow of product at the dispenser when it detects a leak. The Bravo Float and Chain was cleaned, adjusted and retested during the inspection and had passing results. This was corrected on site. This is a minor violation. 315 HSC 25291(e)Water in secondary containment not removed, analyzed, and properly disposed of(pre-Jul 2003). Approximately 2 quarts of liquid was observed in the 87 product STP sump. If water could enter into the secondary containment by precipitation or infiltration, it must be removed and disposed of properly. The service technician removed approximately 2 quarts of liquid from the 87 product STP sump. Ensure that all sumps and annular spaces are maintained free of liquid. This was corrected on site. This is a minor violation. Overall Inspection Comments: The monitoring system certification was conducted on 7/30/2019 and was not fully witnessed by EHD. Looked inside all sumps & UDCs, witnessed sensor functionality and verified spill container 5 gallon capacity.
