A copy of this plan shall be <br /> accessible on site at all times. Immediately locate and retain a copy on site. <br /> This is a minor violation. <br /> 209 CCR 2715(c)(3), 2715(c)(4) Failure to maintain the"Facility Employee Training Certificate"on-site(after October 1, <br /> 2018). <br /> The Facility Employee Training Certificate dated 3/21/19 contained no initial training dates or dates of assuming <br /> responsibility as a facility employee. All site employees must be trained prior to taking responsibility as a facility <br /> employee if after October 1, 2018, or within 30 days of hire if prior to October 1, 2018. The Facility Employee <br /> Training Certificate shall contain accurate information for all employees trained. Update the Facility Employee <br /> Training Certificate to contain the accurate information and submit a copy of the updated form to the EHD. <br /> This is a minor violation. <br /> 301 HSC 25292.1(a)UST system is not operated to prevent unauthorized release, including spills and overfills. <br /> The fuel filters under the dispensers are not located to allow drips and leaks to drain back into the UDC. The <br /> underground storage tank system shall be operated to prevent unauthorized releases. Immediately provide <br /> approved secondary containment for the filters under permit and inspection by the EHD. <br /> This is a Class II violation. <br /> 302 CCR 2630(d), 2641(a) Monitoring equipment is not installed or maintained to detect a leak at the earliest <br /> opportunity. <br /> The L4 diesel annular sensor went into high liquid alarm on 7/11/18. The designated operator found the sensor still <br /> in alarm on 7/26/18. No work orders or maintenance records were found addressing this alarm. A service <br /> technician was on site and corrected the issue on 7/31/18. All monitoring equipment shall be maintained to be able <br /> to detect a leak at the earliest possible opportunity. Ensure that all future alarms are appropriately addressed <br /> immediately. <br /> This is a Class II violation. <br /> 305 HSC 25290.1,25290.2, 25291 Failed to have secondary containment or secondary containment not tight. <br /> The fuel filters under the dispensers are not located to allow drips and leaks to drain back into the UDC. Secondary <br /> containment shall be provided for all single-walled UST components.
