Current financial responsibility documents have not been submitted to the EHD. The financial responsibility documents submitted under the Certification of Financial Responsibility section of CERS are dated 3/21/18. The document under the Letter from the Chief Financial Officer section is dated 3/30/17. Current financial responsibility documents are required to be submitted annually. Immediately log into the California Environmental Reporting System(CERS) at http://cers.calepa.ca.gov/, and upload the required documents. This is a Class II violation. 106 CCR 2632(d)(2), 2634(e) Failed to have an approved UST response plan. The response plan is not current and/or not approved by the EHD. The response plan contains outdated personnel information. The response plan must be uploaded to the California Environmental Reporting System (CERS). Immediately log into CERS, upload the completed form, and submit for review by the EHD. Ensure a copy is available for inspection at any time. This is a minor violation. 113 CCR 2638(c), 2638(d) Failure to submit"Monitoring System Certification Form"within 30 days of completion of the test. Annual monitoring system certification, leak detector testing, and spill container testing was performed on April 23, 2018 and a test report was submitted on July 10, 2018, two months late. A copy of the test report must be submitted within 30 days of the tests. Take all necessary precautions to ensure testing and submittal of test reports are performed in a timely manner. This is a repeat violation, Class II. 201 CCR 2712(b) Failure to maintain monitoring, maintenance, and follow up action records. The 2017 monitoring system certification test results were not found on site. These records shall be maintained on site for at least three years. Monitoring records include: (1)date and time of all monitoring or sampling; (2) monitoring equipment calibration and maintenance records; (3) results of any visual observations; (4) results of sample analysis performed a lab or in the field; (5) logs of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6) results of inventory readings and reconciliations. Immediately locate and maintain all maintenance and monitoring records for the last three years on site. This is a minor violation. 202 CCR 2715(c)(3), 2715(c)(4) Failure to maintain the "Facility Employee Training Certificate" on-site (after October 1, 2018). The Facility Employee Training Certificate dated 3/21/19 contained no initial training dates or dates of assuming responsibility as a facility employee. All site employees must be trained prior to taking responsibility as a facility employee if after October 1, 2018, or within 30 days of hire if prior to October 1, 2018. The Facility Employee Training Certificate shall contain accurate information for all employees trained. Update the Facility Employee Training Certificate to contain the accurate information and submit a copy of the updated form to the EHD. This is a minor violation.
