Violation#202 Disposed of caused disposal of HW at an unauthorized point. <br /> On May 11, 2016 Republic Waste delivered a 40-yard container and all of the filter cake product that <br /> was on the ground was placed in this container. On May 25, 2016 Clean Harbor arrived and all of the <br /> filter cake product from the two Republic bins was transferred to the Clean Harbor Transportation bin. <br /> Attached are photo's identifying the areas has been cleaned up. <br /> Violation#204 Disposed or caused disposal of HW at an unauthorized point. <br /> Clean up of the batch tank overflow was completed immediately following the inspection. All of the <br /> contaminated soil was placed in the Filter Cake Hazardous Waste Container.The storm drain lines to <br /> the on-site storm drain retention pond were hydro flushed. The waste was captured at the retention <br /> pond was placed into the hazardous waste bin with filter cake. See attached pictures of area from the <br /> batch tank to the North storm drain inlet, and the retention pond. <br /> The Republic 40-yard bin that had a hole in the bottom was replaced with a lined container from Clean <br /> Harbor. The City now uses a rental bin for up 90 days of storage of hazardous waste (filter cake) at <br /> LAWTF. See picture of Clean Harbor storage bin. <br /> Violation#301 Facility not maintained to minimize the release of a hazardous waste. <br /> It should be noted the filter press sits over the sanitary sewer and not the storm drain. <br /> Clean up of the batch tank overflow was completed immediately following the inspection. All of the <br /> contaminated soil was placed in the Filter Cake Hazardous Waste Container.The storm drain lines to the <br /> on-site storm drain retention pond were hydro flushed.The waste was captured at the retention pond <br /> was placed into the hazardous waste bin with filter cake. See picture of Clean Harbor storage bin. <br /> The contaminated soil was removed and placed in the hazardous waste container that is picked up by <br /> Clean Harbor. <br /> The one-gallon container of Graffiti Buster was completely used and is no longer on site. <br /> Violation#401 Failed to store hazardous waste in a container that is in good condition. <br /> Graffiti buster, paint thinner, and by-lux bright (safety) red have been cleaned up and removed from <br /> site. <br /> Violation#403 Failed to keep hazardous waste container closed except when adding or removing <br /> hazardous waste. <br /> The bin, which stores the filter press cake is kept closed at all times with the exception of loading or <br /> unloading. See attached photo of closed hazardous waste storage container. <br /> Violation#404 Failed to inspect hazardous waste storage areas at least weekly. <br /> Hazardous waste containers are inspected daily as part of the Operators daily operations of the water <br /> treatment facility. <br /> Violation#407 Contaminated container not marked with date emptied or managed within one year <br /> of being emptied. <br /> The empty herbicide barrels had been triple rinsed with the rinse water placed in spray rig and barrels <br />
