Financial responsibility documents have not been submitted to the EHD. -The most recent financial responsibility documents on CERS are dated 1/15/2018. Current financial responsibility documents are required to be submitted annually. Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, and upload the required documents. This is a minor violation. 105 CCR 2632(d)(1), 2634(d) Failed to have an approved UST monitoring plan. The monitoring plan is not current and/or not approved by the EHD. -Monitoring plans for all three tanks incorrectly lists the monitoring panel as a TLS 350. The panel is a TLS 350 Plus. -The monitoring plans for all three tanks incorrectly state the site is keeping tank integrity test results. tank gauging test results, and ATG test results. The monitoring plan must be uploaded to the California Environmental Reporting System (CERS). Immediately log into CERS, make the necessary changes, and submit for review by the EHD. This is a minor violation. 107 CCR 2632(d)(1)(C) Plot plan/site map not submitted or failed to completely show where monitoring is performed. An accurate UST Monitoring Site Plan was not submitted. -Annular sensors were not marked on map. A site plan must be submitted identifying the locations where monitoring will be performed. Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/and upload a copy of the UST Monitoring Site Plan. This is a minor violation. 202 CCR 2715(c)(3), 2715(c)(4) Failure to maintain the "Facility Employee Training Certificate" on-site (after October 1, 2018). -The Facility Employee Training Certificate dated April 9, 2019 indicated Gagan Singh and Drime Punay were trained on 10/21/2018. Training records for 10/21/2018 were not found onsite. All site employees must be trained prior to taking responsibility as a facility employee if after October 1, 2018, or within 30 days of hire if prior to October 1, 2018. The Facility Employee Training Certificate shall contain accurate information for all employees trained. Update the Facility Employee Training Certificates to contain the accurate information and submit copies of the updated forms to the EHD. This is a minor violation.
