This <br /> form must be uploaded to the California Environmental Reporting System (CERS). Immediately log into the <br /> California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov and upload the Underground <br /> Storage Tank Statement of Understanding and Compliance Form for this facility. <br /> This is a minor violation. <br /> 209 CCR 2716(a-e) Failure to conduct the designated UST operator visual inspection at least once every 30 days. <br /> The designated operator failed to check and note all required items on the visual inspection reports. <br /> -The 6/22/19, 5/24/19, 2/25/19, 1/25/19, 12/24/18, 11/23/18 and 10/25/18. inspection reports left blank section V <br /> and VI <br /> The 10/25/18, 3/26/19, 4/25/19, 7/17/19, 8,12,19, and 9/11/19 inspection reports left blank section VI. <br /> During the visual inspection, the designated operator shall check that all required testing and maintenance for the <br /> UST system have been completed and shall check all required items on the report. Ensure that designated <br /> operators performing visual inspections at this facility are including all of the required information on the reports. <br /> This is a minor violation. <br /> 308 CCR 2632(c), 2634(d), 2636(f) Failed to maintain a continuous monitoring system that activates an audible and <br /> visual alarm. <br /> The 87 (center) STP sump sensor failed to activate an audible and visual alarm when tested and, therefore, the <br /> tank is not monitored. The interstitial space shall be monitored with a monitoring system connected to an audible <br /> and visual alarm system. Immediately contact a properly licensed, trained, and certified contractor to replace the <br /> failed component with a functional component(LG 113-x listed, if applicable), and obtain a permit within one <br /> business day from the EHD, if needed. <br /> This is a minor violation. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by October 27, 2019. <br /> Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate ($152).
