Use this form to report all operations during drilling and testing of the well or during redrilling or altering the casing,plugging,or abandonment,with the dates thereof. Include such items as hole size,formation test details, amounts of cement useo, top and bottom of plugs, perforation details,sidetracked junk,balling tests,and initial production data. Date 2008 4/14 MIRU workover rig. Found the well with no pressure on tubing and casing. Took on water. Pumped 43 barrels of fresh water down 4-1/2" casing and pumped down annulus. Filled the well pressured it up to 500 psig. Removed X-mas tree. Inspected tubing hanger. Installed prod4oion tree back on well. Secured rig. 4/15 Removed X-mas tree. Installed 7-1/16", 5M BOP. Nippled up and tested BOP to 2000 psig. Unset packer. Removed casing hanger. Circulated and POOH. Laid down 4-1/2" casing and packer(84 joints of casing and I PIP packer). Measured, drifted and pickup 2 3/8"4.7#,J-55, EUE, used tubing. RIH with tubing to 2972' and tagged hard fill. Attempted to circulate out fill unsuccessfully. EPA declared that it would be deep enough for cement plug. Pulled 5 stands and secured rig. 4/16 RIH and tagged bottom at 2972'. Circulated hole clean. Rigged up MMI Cementers. Cement Plu¢ na, 29721. Equalized 150 sacks of Class "G" cement premixed with 2% CaC 12 at 2972'. CIP @ 10:00 am. Pulled up to 2000' and reverse circulated clean. Pulled 5 stands and waited on cement 4 hours. RIH and tagged top of hard cement at 2269'. Circulated and rigged up MMI Cementers. Cement Plug(u, 2269'. Equalized 240 sacks of Class "G" cement premixed with 0.1% retarder at 2269'. Pulled up to 950' and reverse circulated clean. Pulled 5 stands and secured rig. 4/17 RIH and tagged top of cement at 1067'. POOH laying down excess tubing. Rigged up MMI Cementers. Cement Plug (7a, 1067'. Equalized 191 sacks of Class "G" cement at 1067' (the first 50 sacks premixed with CaC12, the remaining 141 sacks neat Class "G" cement). POOH laying down tubing. Waited on cement 4 hours. RIH with 6 joints of tubing and tagged top of cement at 165'. Cement Plug (a)a 165'. Equalized 32 sacks of cement. Pulled out laying down tubing and filling hole with cement. Removed BOP. Pigged down the rig, Cleaned out washout pit and secured rig. 4/18 Cut off wellhead. Dug out cement on top of cellar and dug 5' below ground level. Found the cement 5' below. Mixed and poured 2 sacks of cement and welded 1/2" steel plate on 7" casing five feet below ground level. Back filled cellar. Cleaned pit. Prepared to move the rig. 4/21 Loaded out and moved the rig off location.
