Victor Rd., Lodi, CA 95242 <br /> The San Joaquin County Environmental Health Department (EHD) has completed the review of the <br /> application for a permit to replace the dispenser, install conversion frames, and perform a cold start of <br /> the monitoring panel on the underground storage tank (UST) system at the above noted address. The <br /> permit has been approved with the following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control District for <br /> any applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval prior to <br /> commencing work. All new addenda supersede the previous ones. <br /> 3. Contact Stacy Rivera at (209) 468-3440 to arrange the following inspection appointments (a <br /> 48-hour notice is required for inspection appointments). Please be aware that inspections for <br /> emergency work must be scheduled within five days of permit approval. <br /> a. Sensor functionality: Functionality of the monitoring system must be verified to ensure that <br /> monitoring has been restored to meet the requirements of Title 23 California Code of <br /> Regulations and the Health & Safety Code. A copy of the alarm history shall be printed <br /> prior to performing the "cold start" of the Veeder Root system and made available at the <br /> time of EHD inspection. <br /> b. Verification of the dispenser footprints and filters contained by the under dispenser <br /> containment sumps (UDCs) <br /> c. Visual inspection of the piping within the dispensers <br /> 4. Any individual performing the work of a service technician must meet the following <br /> qualifications: <br /> a. Possess or be employed by a person who possesses one of several permissible valid, <br /> current contractor's licenses issued by the Contractors State License Board (may be issued <br /> to the installation company), or a Tank Tester's license issued to the individual by the State <br /> Water Board, and <br /> b. Possess or work under the direct and personal supervision of an individual physically <br /> present at the work site who possesses a valid, current California UST Service Technician <br /> certificate issued by the International Code Council (ICC), and <br /> c. Possesses a valid, current certificate of training from the manufacturer(s) of the <br /> component(s) being tested, repaired, or serviced. <br /> 1868 E.
