UST Tank Information forms are not current and accuate in CERS. -The diesel tank form states the vent lines are single-walled. The 87 and 91 tank forms state the vent lines are double-walled. All three forms accurately state there is no vent sump. -The 87 and 91 tank forms state the vapor recovery piping is double-walled. Any change of information must be updated in CERS within 30 days of the changes. Immediately log into CERS, update the required information, and submit for review by the EHD. This is a repeat violation, Class II. 209 CCR 2716(a-e) Failure to conduct the designated UST operator visual inspection at least once every 30 days. The designated operator inspections are being performed more than 30 days apart. -Inspection on 10/19/18, next done 11/19/18, 31 days later -Inspection due date 11/18/18, next done 12/19/18, 31 days apart -Inspection due date 12/18/18, next done 1/21/19, 34 days apart -Inspection due date 1/17/19, next done 2/21/19, 35 days apart The designated operator visual inspections shall be conducted within 30 days or less of the previous inspection. Ensure that all designated operator inspections are performed within the required time frames. NOTE: The next designated operator inspection is due on or before 3/18/19. The designated operator failed to check and note all required items on the visual inspection reports. -The 10/19/18, 11/19/18, 12/19/18, 1/21/19, and 2/21/19 inspection reports contained no CERS ID. -The 10/19/18, 11/19/18, 12/19/18, 1/21/19, and 2/21/19 inspection reports did not address the 'NA's in Section III. -Section V was left blank on the 1/21/19 inspection report. -The 11/19/18, 12/19/18, 1/21/19, and 2/21/19 inspection reports all marked that each follow up action from the previous report was completed appropriately. The overfill prevention testing was marked as not completed on each of the previous reports and had not yet been corrected. During the visual inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and shall check all required items on the report. Ensure that designated operators performing visual inspections at this facility are including all of the required information on the reports. This is a repeat violation, Class II. 315 HSC 25291(e)Water in secondary containment not removed, analyzed, and properly disposed of (pre-Jul 2003). Liquid was observed in the diesel STP sump. If water could enter into the secondary containment by precipitation or infiltration, it must be removed and disposed of properly. The service technician removed the liquid from the sump. Ensure that all sumps and annular spaces are maintained free of liquid. This was corrected on site. This is a repeat violation, Class II.
