State of California <br /> Department of Health Services <br /> Nooj <br /> Memorandum MPR '�� <br /> Jim Tjosvold <br /> To : Senior Waste Management Engineer�NN'p��M\� ISEDare March 8, 1989 <br /> Subject: MCCORMICK & <br /> BAXTER WELL <br /> ABANDONMENT <br /> From Jeff Van Slooten �.V� <br /> Associate Hazardous Materials Specialist <br /> The Department of Health Services (DHS) received a telephone call <br /> from San Joaquin Local Health District (SJLHD) on Monday, March <br /> 6 , 1989 . CH2M-Hill filed for a permit to abandon well A-9 at <br /> McCormick and Baxter (M&B) and SJLHD was confirming whether a <br /> workplan had been submitted and approved by the State. <br /> The State was informed that M&B was planning to abandon the well, <br /> but had never received any submittal describing the procedure. <br /> CH2M-Hill telefaxed the workplan (attached) on March 7 for State <br /> review and approval . <br /> DHS received verbal approval from the Regional Water Quality <br /> Control Board on the workplan. A DHS staff geologist (Richard <br /> McJunkin) has reviewed the workplan and has also approved the <br /> procedure. DHS's only concerns are that the proper ratio is used <br /> for the grout and the grout is thoroughly mixed. These <br /> specifications will be transmitted to SJLHD. <br />
