During your visit, you brought a few items to our attention . <br /> We have noted each issue below with our plan to address them . <br /> Violation : 109 CCR 2637 ( aA ), 2712 Failed to test secondary containment, or failed to maintain test <br /> records for 36 months . Secondary containment test for SR0078543 was performed on 4/11/2018 and six <br /> month testing was due by October 2018 . Testing was completed on 11/15/2018, 1 month late . <br /> Secondary containment testing is required six months after installation and at least every 36 months <br /> after . Take all necessary precautions to ensure testing is performed in a timely manner . This is a Class II <br /> violation . <br /> Response : This has been noted and we will respond accordingly in the future . <br /> Violation : 306 CCR 2630 ( d ), 2641 ( a ) Monitoring equipment is not installed or maintained to detect a <br /> leak at the earliest opportunity. The site is using flex piping . Test boots were not pulled back, but drain <br /> valves were unplugged . Hoses were observed connected to the test boot drain valves on the product <br /> pipes in the UDCs . Several of the drain valves were pointed up instead of down . Several hoses were <br /> observed with loops and upward sections . The hoses were not positioned and maintained for the <br /> sensors to detect a leak at the earliest opportunity. Monitoring equipment shall be maintained to be <br /> able to detect a leak at the earliest possible opportunity . Immediately make all necessary changes to <br /> ensure that all monitoring equipment is maintained and located to detect a leak at the earliest <br /> opportunity. This is a Class II violation . <br /> Response : This was corrected on site during your visit. This was corrected by Alltech Petro Inc . on <br /> 09/26/2019 . The information is located on page 1 of the monitoring certification form . <br /> Violation : 313 HSC 25290 . 1, 25290 . 2, 25291 Failure to construct, operate, and maintain primary <br /> containment as product-tight . Fuel drips were observed under dispenser 11/ 12 . Dark staining was <br /> observed under the drips . All primary containment for the UST system must be product tight . <br /> Immediately have a properly licensed, trained , and certified contractor repair or replace the failed <br /> component (s ) , under permit and inspection of the EHD if necessary . This is a minor violation . <br /> Response : Elite IV Contractactors came out to inspect pump 11 for leaks and found no leak at this time . <br /> Found cutting oil on meter. No gas leaks were present at the time of inspection . <br /> Violation : 319 CCR 2635, 2637 . 1, 2665 , 2712 No spill bucket, failed to contain 5 gallons, no means to <br /> keep it empty, or failed to be tested . The additive and 87B spill containers failed when tested . <br />
