SA N J O A Y U I N Environmental Health Department <br /> COUNTY <br /> Underground Storage Tank Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> COUNTRY MARKETPLACE 1 1789 W CHARTER WAY STOCKTON June 07 2019 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I, CLASS Il,or MINOR-Notice to Comply) <br /> Item # Remarks <br /> 202 CCR 2715(c)(3), 2715(c)(4) Failure to maintain the "Facility Employee Training Certificate" on-site(after October 1, <br /> 2018). <br /> The Facility Employee Training Certificate dated 4/19/19 was missing dates for the following sections: <br /> -initial training <br /> -assuming responsibility as a facility employee <br /> All site employees must be trained prior to taking responsibility as a facility employee if after October 1, 2018, or <br /> within 30 days of hire if prior to October 1, 2018. The Facility Employee Training Certificate shall contain accurate <br /> information for all employees trained. Update the Facility Employee Training Certificate to contain the accurate <br /> information and submit a copy of the updated form to the EHD. <br /> This is a Class II violation. <br /> 204 CCR 2712(i) Current monitoring plan approved by the EHD not found on site. <br /> An approved copy of the monitoring plans were not available for inspection. A copy of these plans shall be accessible <br /> on site at all times. Immediately locate and retain a copy on site. <br /> This is a repeat violation, Class II. <br /> 209 CCR 2716(a-e) Failure to conduct the designated UST operator visual inspection at least once every 30 days. <br /> The designated operator failed to check and note all required items on the visual inspection reports. <br /> -Section V on the 2/22/2019 and 3/22/2019 inspection reports were missing owner/operator's description of follow-up <br /> actions about scheduling an overfill prevention test. <br /> During the visual inspection, the designated operator shall check that all required testing and maintenance for the UST <br /> system have been completed and shall check all required items on the report. Ensure that designated operators <br /> performing visual inspections at this facility are including all of the required information on the reports. <br /> This is a minor violation. <br /> 306 CCR 2630(d), 2641(a) Monitoring equipment is not installed or maintained to detect a leak at the earliest opportunity. <br /> The flexible piping and jumper hoses were observed in both 87 and 91 products in all UDCs preventing a leak to be <br /> detected at the earliest opportunity. Any potential release from the primary containment into the secondary <br /> containment in the 87 product and 91 product piping does not flow into a UDC collection sump to detect a leak at the <br /> earliest opportunity. Immediately contact a properly licensed, trained, and certified contractor to ensure that any <br /> releases in the 87 product and 91 product piping flows into the nearest sump for a leak to be detected at the earliest <br /> possible opportunity.
