Ralph Hayes & Son, Inc. <br /> Spill Reporting Procedures <br /> RHS has prepared an Emergency Response Plan to be activated in the event of a spill. In <br /> general, the following requirements must be adhered to in the event of a release: <br /> Notify the Fleet Manager regardless of quantity released and provide the following information: <br /> 1. Date and time of discharge; <br /> 2. Type of substance released; <br /> 3. Estimate of quantity released; <br /> 4. Source of discharge; <br /> Step 1: 5. A description of all affected media; <br /> 6. Status of release including actions being uscd to stop,remove and mitigate the effects of the <br /> discharge; <br /> 7. Whether an evacuation may be needed; <br /> 8. Individuals or organizations contacted; <br /> 9. Any injuries;and <br /> 10. Any damage to property. <br /> Step 2: Notify onsite personnel of evacuation procedures,if necessary. <br /> Step 3: Notify Emergency Facilities--Dial 911 if public is in imminent danger. <br /> Step 4: Notify the Governor's Office of Emergency Services at 800-852-7550 <br /> Once the spill is under control,notify the following within 24 hours if quantity of oil spilled is greater <br /> than 42 gallons: <br /> 1.
