SAN -IOAQUIN Environmental Health Department <br /> --COUNTY <br /> Underground Storage Tank Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> CHEVRON#208117** 755 S TRACY BLVD, TRACY February 07, 2019 <br /> Item# Remarks <br /> 109 CCR 2637(a-d), 2712 Failed to test secondary containment, or failed to maintain test records for 36 months. <br /> Under dispenser containments 1/2, 3/4, 5/6, 7/8 and 9/10 were installed under service request#SR0078631 on <br /> 4/12/18 and six month testing was due by October 2018. Testing has not been completed and is 3 months past <br /> due. Secondary containment testing is required at the time of installation and six months after installation and at <br /> least every 36 months after. Immediately schedule secondary containment testing for UDC 1/2, 3/4, 5/6,7/8, 9/10, <br /> 87 product and 91 product. Provide 48 hours notification to the EHD. <br /> This is a Class II violation. <br /> 124 CCR 2637.2(d&e), 2665(a&b)"Overfill Prevention Equipment Inspection Report Form"not submitted within 30 days <br /> of inspection. <br /> overfill prevention equipment inspection report was performed on 11/21/18 and a test report was submitted on <br /> 1/18/19,28 days late. A copy of the test report must be submitted within 30 days of the tests. Take all necessary <br /> precautions to ensure testing and submittal of test reports are performed in a timely manner. <br /> This is a minor violation. <br /> 209 CCR 2716(a-e) Failure to conduct the designated UST operator visual inspection at least once every 30 days. <br /> On the 10/31/18 designated operator(DO)inspection report under: <br /> -Section VI <br /> The OWNER/OPERATOR failed to provide a name, signature, and date within 48 hours of receiving the DO report <br /> under the ACKNOWLEDGMENT OF COMPLIANCE ISSUES section.The designated operator(Vivian Vargas)and <br /> not the operator signed the 10/31/18 DO report on 10/31/18. Ensure that the owner/operator of the facility reviews <br /> section III and signs section VI of the DO report. <br /> -Section X <br /> The DO failed to mark NO under the question"Has the overfill prevention equipment inspection been completed <br /> within the past 36 months?"The overfill prevention equipment inspection was conducted on 11/21/18. <br /> During the monthly inspection,the designated operator shall review the alarm history for the previous month, check <br /> that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with <br /> documentation taken in response to any alarms to the monthly report. Ensure that designated operators performing <br /> inspections at least once every 30 days at this facility and are including all of the required information on the reports. <br /> This is a minor violation. <br /> 313 HSC 25290.1, 25290.2, 25291 Failure to construct, operate, and maintain primary containment as product-tight. <br /> Seepage was observed in the 91 product union in Under dispenser containment 1/2, 3/4 and 9/10, indicating a leak <br /> in the primary piping. All primary containment for the UST system must be tight.
