Ha7ardotjs Waste Documentation, continued <br /> A "Hazardous Waste Manifest" is the legal shipping document that must accompany every <br /> shipment of hazardous waste. When Stericycle makes a pickup at your store, they are <br /> required to leave the Manager-On-Duty a completed manifest. <br /> Note: You may receive more than one manifest per pickup. <br /> They will also provide a "Land Disposal Restriction" (LDR) form, which is a document that - <br /> ensures that wastes are properly treated prior to disposal. The LDR form must be <br /> maintained together with the manifest. • <br /> It is the Manager-On-Duty's responsibility to ensure that the manifest and LDR has been <br /> completed correctly for each shipment of hazardous waste. Stericycle will complete the <br /> required information on CVS's behalf, but the Manager-On-Duty must verify it for accuracy. <br /> All manifests and LDR forms provided by Stericycle must be legible. If the manifests and/or <br /> LDR forms are not legible, ask the Stericycle technician to reprint another copy. <br /> The Manager-On-Duty must review the LDR form and make sure the waste codes listed on <br /> manifest match those on the LDR form.
