San Joaquin County <br /> Environmental Health Department <br /> 1868 East Hazelton Avenue,Stockton,California 95205-6232 <br /> Telephone:(209)468-3420 Fax:(209)468-3433 Web:www.siaov.org/ehd <br /> RCRA Large Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> CVS Pharmacy#9830 6632 Pacific Ave, Stockton August 20,2018 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 118 CCR 66265.16(a-c)Failed to complete training on hazardous waste management and emergency response <br /> procedures. <br /> -A Revlon Cr6me Gel Eye Liner was found in a Universal Waste tote,although it was labeled as"AF"for the <br /> Flammables tote. <br /> -A Dove Men+Care Shampoo was found in the Oxidizers tote, although the Zebra Scanner stated to place the <br /> shampoo in the Toxic Waste tote. <br /> -Dawn,Ajax,and Palmolive dish soaps were placed in the Flammables tote,although the Zebra Scanner stated to <br /> place them in in the Toxic Waste tote. <br /> -Four CVS Sport spray sunscreen cans and five CVS Kids spray sunscreen cans were found in a Toxic Waste tote, <br /> although the Zebra Scanner stated to place the cans in the Flammables tote. <br /> -When asked why an electronic light switch was found in a Toxic Waste tote,the store manager,Jon Alley,stated <br /> that although he thought the light switch should have been placed in the Universal Waste tote, he continued to place <br /> the light switch in the Toxic Waste tote because he was prompted by the Zebra Scanner to do so.Jon did not know <br /> who to call to determine which hazardous waste(HW)container the light switch should be placed in. <br /> -An open unlabeled grey tote full of HW in StrongPak bags and an open red basket full of un-bagged and unlabeled <br /> HW were found on the ground behind the registers at the front of the store. <br /> -Nicotine products expired June 2018 and July 2018 were found on the shelves behind the registers,for sale. <br /> -On the Weekly Hazardous Waste Inspection Log done on 8/19/18 by Jon, he indicated there were 18 containers of <br /> HW that were properly labeled,closed,and not passed the 90-day storage limit.However, initially only 14 HW <br /> containers were observed in the HW waste storage area.Jon stated the four missing HW containers were at the <br /> front of the store behind the registers,however there was only one grey tote of HW in StrongPak bags behind the <br /> registers.The tote behind the register was found open.Jon found the other three grey totes that contained HW in <br /> StrongPak bags closed and adjacent to the hazardous waste storage area.The four grey totes were not labeled and <br /> contained HW of varying HW properties.According to Jon, he had processed the HW in the grey totes,but did not <br /> have time to put the HW in the correct HW storage container. <br /> -Two Beauty 360 Advanced Nail Polish Remover Gels were found without UPCs inside StrongPak bags inside an <br /> unlabeled container.When asked how he makes a HW determination for products without a UPC,Jon stated he <br /> pulls and scans a similar product from the floor. <br /> -When an Oxi Clean Laundry Detergent was found leaking with a broken cap inside a StrongPak bag inside an <br /> unlabeled container,Jon Alley stated that he threw away the debris used to clean-up the spill. <br /> -A large StrongPak bag full of hazardous waste in smaller StrongPak bags was found on top of a red basket, <br /> located on the 2nd floor of the store,although Jon stated there was no storage of hazardous waste on the 2nd floor. <br /> -A cracked leaking container of Tide Original laundry detergent was found sitting over a floor drain adjacent the <br /> mop sink in the store's back storage room. <br /> Personnel who handle hazardous waste shall be trained within six months of assignment and take part in an annual <br /> review of the initial training received. At a minimum,the training program shall be designed to ensure that facility <br /> personnel are able to respond effectively to emergencies by familiarizing them with emergency procedures, <br /> emergency equipment,and emergency systems, including where applicable: <br /> 1. procedures for using, inspecting, repairing,and replacing facility emergency and monitoring equipment; <br /> 2. key parameters for automatic waste feed cut-off systems; <br /> 3. communications or alarm systems; <br /> 4. response to fires or explosions; <br /> 5. response to ground-water contamination incidents;and <br /> 6. shutdown of operations. <br /> The owner or operator shall maintain the following documents and records at the facility: <br /> 1. the job title for each position at the facility related to hazardous waste management, and the name of each <br /> employee filling each job; <br /> FA0015387 PRO536194 SCO01 08/202018 <br /> EHD 22-03 Rev.09/22/16 Page 8 of 12 RCRA Large Quantity Hazardous Waste Generator OIR <br />
