The designated operator failed to check and note all required items on the visual inspection reports. -The 7/11/19 inspection report t left blank section VI. -The 04/12/19 inspection report was signed by the owner/operator on 04/15/19, 24 hours late. The owner/operator is expected to sign the designated operator report within 48hours from the date of the report. -The 11/14/18 inspection report was signed by the owner/operator on 11/30/18., 14days late. The owner/operator is expected to sign the designated operator report within 48hours from the date of the report -The 10/20/16 inspection report was signed by the owner/operator on 10/31/18, nine days late. The owner/operator is expected to sign the designated operator report within 48hours from the date of the report During the visual inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and shall check all required items on the report. Ensure that designated operators performing visual inspections at this facility are including all of the required information on the reports. This is a minor violation. 319 CCR 2635, 2637.1, 2665, 2712 No spill bucket, failed to contain 5 gallons, no means to keep it empty, or failed to be tested. The 91 product direct bury spill bucket failed when tested. All USTs shall be equipped with a spill container that has a minimum capacity of five gallons and a drain valve that allows drainage of the collected spill into the primary container or provide a means to keep the container empty. Immediately contact a properly licensed, trained, and certified contractor to install an approved spill container under permit and inspection of the EHD. If a spill container can't be installed immediately, there is a possibility that the 91 UST system may be red tagged to prevent fuel inputs. This is a minor violation. 320 CCR 2635, 2637.2, 2665, 2712 Failure to comply with the installation, inspection, or record keeping of the overfill prevention. Overfill prevention testing was conducted on 10/25/18. This testing was required to be conducted by October 13, 2018. Ensure all future testing is conducted within required time frames. This is a Class II violation. Overall Inspection Comments: Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement documenting the corrective actions that have been or will be taken for each violation, and any supporting paperwork, by August 17, 2019.
