Note: This is a repeat violation from the inspection on February 3, 2006. <br /> Class II Violations <br /> Violation #24-Failed to manage incompatible wastes in containers properly 66265.17 CCR. <br /> At time of inspection, facility was storing mixed waste aerosol cans, in FL totes, without any knowledge <br /> of compatibility. <br /> Facility was also storing mixed wastes, on the sorting cart, including containers of Murphy's oil, motor <br /> oil, bleach, and antifreeze, some of which were open, broken, and or leaking, for an undetermined <br /> amount of time, without any knowledge of compatibility. <br /> Immediately, segregate incompatible wastes. All incompatible wastes must be managed <br /> and stored separately. <br /> Violation #27-Failed to keep containers of HW closed except when adding/removing <br /> hazardous waste 66265.173(a) CCR. <br /> At time of inspection, facility was storing mixed wastes, on the sorting cart, including, containers of <br /> Murphy's oil, motor oil, bleach, and antifreeze, some of which were open, broken, and or leaking. <br /> Hazardous waste containers must be kept closed at all times unless actively adding or removing <br /> hazardous waste. <br /> Immediately, close all storage containers of hazardous waste unless actively adding or <br /> removing hazardous waste. <br /> Violation #50-Stored Hazardous Waste onsite greater than 180 days CCR66262.34(d). <br /> At time of inspection, start accumulation date on the hazardous waste stored in the pharmacy was <br /> listed as 2/1/09. <br /> Immediately, have this waste removed by a registered hazardous waste hauler, under manifest, <br /> and submit a copy to this office. <br /> Violation #59-Generator manifest copies not sent to the Department of Toxic Substances <br /> Control (DTSC) within 30 days 66262.23(a) (4) CCR. <br /> At time of inspection: <br /> - Blue copy (generator copy) of a hazardous waste manifest, number 24688632, dated 5/06 was <br /> seen. <br /> - Generator copy of the following manifests were not submitted to the DTSC within 30 days of <br /> shipment: <br /> 000273560WAS 1/19/10 <br /> 000273116WAS 12/8/09 <br /> 000272772WAS 10/15/09 <br /> 000247067WAS 07/09 <br /> 000226117WAS 08/20/09 <br /> 000186159WAS 05/28/09 <br /> Generator copies must be sent to the department within 30 days of each shipment of hazardous waste. <br /> Within 30 days, May 20, 2010, submit copies to the DTSC and provide this office with proof <br /> of submittal. <br /> Violation #60-Failed to keep signed copy for 3 years 66262.40(a) CCR.
