There must be at least one <br /> emergency coordinator on site or on call to coordinate emergency response measures, and the following <br /> information must be posted by a phone: the name and phone number of the emergency coordinator; location of fire <br /> extinguishers, spill control equipment, and if present,fire alarm; and the phone number of the fire department, <br /> unless the facility has a direct alarm. Immediately appoint an emergency coordinator and post the required <br /> information by a phone. A form was provided to Tariq Khan that can be used for this purpose. <br /> This is a Class II violation. <br /> 106 CCR 66262.34(d)(2) Failed to train employees on waste handling and emergency procedures. <br /> At the time of inspection, it could not be demonstrated (not necessarily documented)that employees who handle <br /> hazardous waste were properly trained. The following observations were made in the facility's back storage area at <br /> the time of inspection: <br /> -A Hazardous Waste (HW)Aerosols, Flammables and Combustibles tote with accumulation start date 8/4/18 was <br /> found lacking the name of the generator and the contents marked on the HW label (IMG 2187). Tariq added the <br /> name of the generator and marked the contents on the HW label at the time of inspection (IMG 2193). <br /> -A container of a blue liquid cleaner/detergent was found in an open bag leaking onto the floor(IMG 2196-2197). <br /> The assistant manager, Sheila Thomas, cleaned the spill using absorbent pads and placed the container and used <br /> absorbent into a bag labeled hazardous waste and into a HW Corrosive Bases tote at the time of inspection. <br /> -An open cardboard box contained 12 bottles of White Rain Body Wash,five of which were visibly damaged <br /> including two which were visibly open with leakage on the outside of the bottles(IMG 2207). <br /> The items listed above were identified as waste by Tariq at the time of inspection. <br /> -An open black crate was found labeled as"Open Packages"on a shelf in the facility's back storage room, and <br /> contained the following items(IMG 2203): <br /> -A container of Kendall Liquid Titanium Motor Oil (IMG 2198) <br /> -Five boxes of Natureplex Acne Treatment Clear Gel (IMG 2198-2020).These items were identified as hazardous <br /> waste by Tariq and were placed in hazardous waste bag and into a HW Corrosive Bases tote at the time of <br /> inspection. <br /> -An open tube of toothpaste (IMG 2204).This item was identified as waste by Tariq at the time of inspection. <br /> -Approximately 10 partially filled aerosol cans(IMG 2205-2206) <br /> -A bottle of White Rain Kids 3 in 1 body wash, shampoo, &conditioner(IMG 2205) <br /> According to Tariq,these items were waiting to be processed under the Manager's Markdown Program or as <br /> hazardous waste.When asked how often items placed in this open crate are processed, Tariq stated between 1-2 <br /> days. However, at the time of inspection the facility did not have any other hazardous waste aerosol cans placed <br /> into the HW Aerosols, Flammables and Combustibles tote since 8/4/18,the accumulation start date, according to <br /> Tariq.Tariq stated that all 10 aerosol cans were placed in the open crate one or two days ago. <br /> The generator must ensure that all employees who handle hazardous waste are thoroughly familiar with proper <br /> waste handling and emergency procedures.
