The modified contingency <br /> plan was missing the name and telephone number of the emergency coordinator,and the location of the spill control <br /> equipment. <br /> REGULATION GUIDANCE: The business operator must post the following information next to telephones or in <br /> areas directly involved in the generation and accumulation of hazardous waste: the name and emergency <br /> telephone number of the emergency coordinator,the location of fire extinguishers and spill control material,and the <br /> telephone number of the fire department unless the facility has a direct alarm. <br /> CORRECTIVE ACTION: Immediately post the required information at the facility. Provide a statement to the San <br /> Joaquin County Environmental Health Department that the correction is complete. <br /> This is a repeat violation,Class II. <br /> 110 CCR 66262.40(a)Failed to keep signed copy of manifests from the designated facility for three years. <br /> OBSERVATION: Designated facility signed and dated copies of Uniform Hazardous Waste Manifests(UHWM) <br /> were not found on-site. The missing manifests are: 013304833FLE(11/7/2019),013304834FLE(11/7/2019), <br /> 013304836FLE(11/7/2019),013304835FLE(11/7/2019),014705940FLE(6/1/2020),014705942FLE(6/1/2020), <br /> and 014705941 FLE(6/1/2020). <br /> REGULATION GUIDANCE: A hazardous waste generator shall keep a copy of each manifest signed in accordance <br /> with section 66262.23(a)for three years or until the generator receives a signed copy from the designated facility <br /> which received the waste.This signed copy shall be retained as a record for at least three years from the date the <br /> waste was accepted by the initial transporter. The hazardous waste generator must submit a copy of the manifest <br /> to the California Department of Toxic Substances Control (DTSC)within 60 days if the destination facility signed <br /> manifest copy is not received from the transporter or designated facility. <br /> CORRECTIVE ACTION: Locate the destination facility signed manifests listed above and maintain in the facility file <br /> for three years from the date of shipping.
