RECEIVED <br /> JUL S 12017 <br /> The following is an itemized list of hazardous waste violations that have not beENVIRONMENTAL <br /> addressed for SUMMIT ROOFING SERVICES as of July 05, 2017. F1GPI VIH DEPARTMENT <br /> Open violations from January 06,2016 inspection <br /> Violation#101 -Managed a hazardous waste without an identification number. <br /> This facility's hazardous waste ID number is inactive. A hazardous waste generator shall not treat,store,dispose <br /> of,transport or offer for transportation, hazardous waste without a hazardous waste ID number. Go to the DTSC <br /> website http://www.dtsc.ca.gov/IDManifest/PERMHWID.cfm for the form to reactivate the ID number. <br /> Immediately reactivate your(EPA OR STATE]ID number and submit evidence to the EHD. <br /> Violation#102-Failed to determine if a waste is a hazardous waste or retain waste analysis on site for 3 <br /> years. <br /> A small quantity of metal fines generated from pipe cutting were observed on and around the metal cutting <br /> equipment.Any person who generates a waste shall determine if the waste is a hazardous waste. Immediately stop <br /> recycling metal fines, make a hazardous waste determination,and manage it according to Title 22 hazardous waste <br /> regulations. Particles 100 microns or smaller must be handled as hazardous waste if the metal is determined to be <br /> a hazardous waste. Submit a statement and supporting documentation explaining how this waste was managed. <br /> The quantity of the generated metal fines was too small for a sample collection at the time of inspection. <br /> Violation#117-Failed to report program data electronically. <br /> A submission to the California Electronic Reporting System(CERS)for the hazardous waste program has not been <br /> made. Beginning January 1,2013,all businesses are required to submit all new(or any changes to existing) <br /> hazardous waste information online to the CERS at http://cers.calepa.ca.gov. Be sure to include your hazardous <br /> waste activity in the Businesses Activities section in CERS in addition to any other relevant activities and required <br /> fields. <br /> Violation#301 -Facility not maintained to minimize the release of a hazardous waste. <br /> An area(located inside the facility),which according to Mr.Asbury was historically used for pump repair had oily <br /> staining around and the ply wood floor area. Mr.Asbury indicated that he no longer repairs pumps onsite and was <br /> planing to clean this area. Multiple open container with oily liquids were noted in the yard area. Facilities shall be <br /> maintained and operated to minimize the possibility of a fire,explosion,or release of hazardous waste to air,soil, or <br /> surface water which could threaten human health or the environment. Immediately fix all leaks and staining, and <br /> manage all clean up waste according to Title 22 hazardous waste regulations. Submit a statement and supporting <br /> documentation explaining how this waste was managed. <br /> Violation#303-Failed to maintain all communication or alarm systems,spill control,or decontamination <br /> equipment. <br /> The onsite eye was station(located on the back wall in the yard area)didn't appear to have been maintained as <br /> required, Mr.Asbury indicated that he didn't think it has been maintained. All facility communications or alarm <br /> systems,fire protection equipment,spill control equipment, and decontamination equipment,where required, shall <br /> be tested and maintained as necessary to assure its proper operation in time of emergency. Immediately begin this <br /> practice. <br /> Violation#403-Failed to keep hazardous waste containers closed except when adding or removing <br /> hazardous waste. <br /> The following open and unlabeled hazardous waste containers were observed onsite at the time of inspection:eight <br /> 5-gallon containers with oily liquids and paint like waste were noted in the yard area(were covered with tarp),one <br /> open and unlabeled container of oily rags and a drain cup full of oil were noted inside another 5-gallon container <br /> stored along with waste(outdated)paints and mastics(covered with tarp)inside the yard area,two open 55-gallon <br /> container of hardened roof asphalt and rain water mixture were noted in the yard area, 3 blue 55-gallon container <br /> with rain water and roofing asphalt mixture were observed(under tarp)in the front yard area. All hazardous waste <br /> containers shall be closed at all times except when adding or removing waste. Immediately close these containers <br /> and ensure all hazardous waste containers are closed when not adding or removing waste. <br /> Page 1 of 2 <br />
