Harding, Stockton Date: Feb 4, 2013 <br />SUMMARY OF VIOLATIONS <br />CLASS I, CLASS II, or MINOR -Notice to Comply) <br />This is a report of the Routine hazardous waste inspection conducted at the referenced facility by San <br />Joaquin County Environmental Health Department (EHD). The following are Minor Violations - Notice to <br />Comply: <br />101. This facility's EPA ID number is assigned to Abraham Cyrus who is not currently operating the <br />facility, and therefore does not have a valid EPA ID number to manage hazardous waste. A hazardous <br />waste generator shall not treat, store, dispose of, transport or offer for transportation, hazardous waste <br />without an EPA ID number. Immediately obtain an EPA ID number to manage hazardous waste and <br />submit evidence to the EHD by 3/5/2013. A form has been provided. <br />NOTES <br />The referenced facility is under receivership of Kevin Singer appointed by Contra Costa County Superior <br />Court on October 16, 2012. The facility is currently operated by Five Star Restaurant Consultants who <br />was retained by Kevin Singer. <br />The following is not a violation: <br />701. Abraham Cyrus failed to dispose two 55 -gallon drums of hazardous waste within 180 days of <br />storage; the drums were disposed by Fiver Star Restaurant after contracting Fremouw Environmental on <br />1/15/13 on manifests #010188954JJK and #010188953JJK .
