SAN J O A Q U I N Environmental Health Department <br /> 0UNIY S <br /> Large Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> JIFFY LUBE #1478 1 1648 E HAMMER LN , STOCKTON May 11 , 2018 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I, CLASS II, or MINOR - Notice to Comply) <br /> Item # Remarks <br /> 120 HSC 25160.2(b)(4) Consolidated manifest receipts failed to contain complete information . <br /> -The consolidated manifest receipt for 5/28/15 with manifest number 014058403JJK was missing the following <br /> required information: the waste code, the amount in pounds, waste minimization statement and destination facility . <br /> -The consolidated manifest receipt for 2/8/17 with manifest number 016766802JK was missing the following <br /> required information: the amount in pounds. <br /> All copies of each receipt shall contain all of the following information: <br /> - The name, address, identification number, contact person, and telephone number of the generator, and the <br /> signature of the generator or the generator's representative; <br /> - The date of the shipment; <br /> - The manifest number; <br /> - The volume or quantity of each waste stream received, its California and RCRA waste codes, the waste stream <br /> type listed in subdivision (c), and its proper shipping description, including the hazardous class and United <br /> Nations/North America (UN/NA) identification number, if applicable; <br /> - The name, address, and identification number of the authorized facility to which the hazardous waste will be <br /> transported ; <br /> - The transporter's name, address, and identification number; <br /> - The driver's signature; <br /> - A statement, signed by the generator, certifying that the generator has established a program to reduce the <br /> volume or quantity and toxicity of the hazardous waste to the degree, as determined by the generator, to be <br /> economically practicable. <br /> Ensure that all future receipts for hazardous waste shipments using a consolidated manifesting procedure contain <br /> all required information. <br /> This is a repeat violation, Class II. <br /> 205 HSC 25250. 7 Failed to manage used oil contaminated with other hazardous waste as hazardous waste. <br /> Peter Gaines, the store manager, stated that used brake fluid was placed in the used oil tank. Immediately stop <br /> mixing used brake fluid with used oil and begin handling all used brake fluid as its own separate waste stream. <br /> Keep disposal records for waste brake fluid for 3 years. <br /> This is a repeat violation, Class II. <br /> 303 CCR 66265 .33 Failed to maintain all communication or alarm systems, spill control, or decontamination equipment. <br /> The eyewash in the basement area was not working because there was no wash water in the reservoir. All facility <br /> communications or alarm systems, fire protection equipment, spill control equipment, and decontamination <br /> equipment, where required, shall be tested and maintained as necessary to assure its proper operation in time of <br /> emergency .
