Box 479 <br /> 23390 E Flood Road <br /> Linden, CA 95236 <br /> Re: Five-Year Permit Review Application for the Scotts Hyponex Regional Composting Facility <br /> (39-AA-0026) <br /> Mr. Bolton, <br /> The Five-Year Permit Review Application for the Scotts Hyponex Regional Composting Facility (39- <br /> AA-0026)was received by the Environmental Health Department(EHD) on January 5, 2007. The <br /> application CIWMB E-1-77, "Application for Solid Waste Facility Permit/Waste Discharge <br /> Requirements," is incomplete and is rejected by the EHD. The following sections must be revised: <br /> 1. Part 2,Facility Description; Section 133, Location of Facility: "NA" is used for this section. <br /> Refer to the maps included with the application or other documentation. The maps included with <br /> the application do not specify the 22 acre permitted compost area. Submit a map which shows <br /> the 22 acre permitted compost area. <br /> 2. Part 2,Facility Description; Section E,Type of Permitted Wastes to be Received: The <br /> Section E5, "Compostable Material',box should be checked instead of Section E15. A <br /> description of the wastes received should be included. <br /> 3. Part 3,Facility Information; Section A,Proposed Change: No box is checked. For a five- <br /> year review with no changes, check box 4 and write in"Five year review with no facility <br /> changes." <br /> 4. Part 3,Facility Information; Section Blc,Facility Information, Information Applicable to <br /> all Facilities,Facility Size(acres): 80 acres is entered on the application. The permitted <br /> compost area is 22 acres as stated in the Solid Waste Facility Permit. <br /> 5. Part 5A, Compliance with California Environmental Quality Act(CEQA): The box for <br /> Environmental Impact Report is checked. The CEQA document for this site is a Negative <br /> Declaration and is included in the Report of Composting Site Information(RCSI),dated February <br /> 1994. Include the SCH# and the posting date. <br /> 6. Part 6A, List of Attachments,Required with all Application Submittals: "NA"was used for <br /> all documents except the location map. A RFI(RCSI), Local Land Use Permit,and an <br /> Environmental Document do exist for this facility. These documents are included in the RCSI. <br /> Include the names, numbers and dates of these documents in this section.
