Hazelton <br /> Ave Stockton, CA 95205 <br /> Re: ADDENDUM TO RTC 6/25/2019 <br /> Dollar Tree#1260 <br /> 3728 East Hammer Lane <br /> Stockton, CA 95212 <br /> Dear Ms. Florido, <br /> In response to your January 10, 2020 request for additional response on the inspection conducted on <br /> May 6, 2019 and the amended report of May 16, 2019, for Store#01260 in Stockton, California, <br /> please see the outline below, and the exhibits which follow, referencing existing procedures, <br /> documentation and/or actions taken: <br /> • Violation 106: As there were management changes at Store 1260, we decided to train/retrain <br /> the management team. We cannot get a clearer copy of the 2019 training signoff, but have <br /> provided the 2020 signoff(EXHIBIT A). The management team was trained using our <br /> current Hazardous Waste Quick Reference Guide. <br /> • Violations 110 and 111: There is no record of a pickup at store#1260 on May 31, 2018 <br /> and no existing manifest no. 011943674FLE in any vendor, company or state records <br /> database system. (We also searched on similar manifest numbers in case something was <br /> transposed, or incorrect in your inspection report.) <br /> • Violation 301: The category for Comet and Ajax powdered cleaners was changed in the <br /> system,per WERCs, from OX to STOU. When we discovered it, WERCs was instructed to <br /> change it back—despite the product(s)not having enough active ingredient to meet the <br /> threshold for oxidizers. We chose to over-classify these products and manage them as <br /> oxidizers. <br /> • Violation 405: My mistake in referring to the developer as conditioner. As determined by our <br /> vendor WERCs' assessment, the various components of the hair color kits do not contain <br /> enough of any chemical (hydrogen peroxide or ammonium peroxide) to meet the thresholds <br /> for an oxidizer or corrosive base classification. According to WERCs, these Dollar Tree <br /> products should actually be handled as state-regulated. There is no need to separate the items <br /> in the kit. <br /> October 10, 2020 pg. 1 <br />
