The following is an itemized list of hazardous waste violations that have not been <br /> addressed for DOLLAR TREE #01260 as of October 09, 2020. <br /> Open violations from May 06, 2019 inspection <br /> Violation#111 -Failed to comply with uniform hazardous waste manifest exception requirements. <br /> The following manifest numbers were found without a signed off copy from the destination facility: <br /> -011386561 FLE (4/20/18)(IMG 5978) <br /> -010740567FLE (8/7/18)(IMG 5976) <br /> -010740566FLE (8/7/18)(IMG 5975) <br /> Manifest number 011943674FLE(5/31/18)was not found on site. <br /> Hazardous waste generators shall retain copies of all manifests for three years or until a copy signed off by the <br /> destination facility is received. The signed off copy shall be retained and made readily available for review for three <br /> years. If the generator did not receive a copy of the manifest with the handwritten signature of the owner or operator <br /> of the facility to which the generator's waste was submitted within 60 days of the date the waste was accepted by <br /> the initial transporter, the generator shall submit a legible copy of the missing manifest,with some indication that the <br /> generator has not received confirmation of delivery. This information shall be submitted to: <br /> DTSC Report Repository <br /> Generator Information Services Section <br /> P.O. Box 806 <br /> Sacramento, CA 95812-0806 <br /> Immediately locate a copy of the missing manifests or prepare and submit the required information to DTSC. <br /> Submit to the EHD a copy of the signed off copy of the manifests or a copy of the information submitted to DTSC. <br /> ❑ This violation was corrected ❑This violation will be corrected by(date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation#301 -Facility not maintained to minimize the release of a hazardous waste. <br /> -A sales shelf in the facility,which held containers of Comet and Ajax bleach-containing cleaning powders,was <br /> found with spillage of the bleach-containing powders (IMG 5950-4). The hand-held scanner indicated the Comet and <br /> Ajax bleach-containing powders should be placed in an STOU HW tote,while the"Products To Be Treated As <br /> Hazardous Waste"guide posted by the HW storage area states"Ajax or Comet Type Cleaner(with bleach)"should <br /> be placed in an OX HW tote(IMG 5928). <br /> -A sales shelf in the facility,which held containers of Ty-D-Bol toilet cleaner,was found with dried up spillage of the <br /> Ty-D-Bol cleaner(IMG 5955-7).The"Products To Be Treated As Hazardous Waste"guide posted by the HW <br /> storage area states"Toilet Bowl Cleaner(without bleach-liquid)"should be placed in a CA HW tote(IMG 5928). <br /> Facilities shall be maintained and operated to minimize the possibility of a fire, explosion,or release of hazardous <br /> waste to air, soil, or surface water which could threaten human health or the environment. <br /> Immediately clean up the spillage and staining from the Comet and Ajax cleaning powders and the Ty-D-Bol toilet <br /> cleaner, and manage according to Title 22 hazardous waste regulations. <br /> Submit a statement and supporting documentation explaining how this waste was managed. <br /> Page 6 of 7 <br />
