The following is an itemized list of hazardous waste violations that have not been addressed for DOLLAR TREE #01260 as of October 09, 2020. Open violations from May 06, 2019 inspection Violation#111 -Failed to comply with uniform hazardous waste manifest exception requirements. The following manifest numbers were found without a signed off copy from the destination facility: -011386561 FLE (4/20/18)(IMG 5978) -010740567FLE (8/7/18)(IMG 5976) -010740566FLE (8/7/18)(IMG 5975) Manifest number 011943674FLE(5/31/18)was not found on site. Hazardous waste generators shall retain copies of all manifests for three years or until a copy signed off by the destination facility is received. The signed off copy shall be retained and made readily available for review for three years. If the generator did not receive a copy of the manifest with the handwritten signature of the owner or operator of the facility to which the generator's waste was submitted within 60 days of the date the waste was accepted by the initial transporter, the generator shall submit a legible copy of the missing manifest,with some indication that the generator has not received confirmation of delivery. This information shall be submitted to: DTSC Report Repository Generator Information Services Section P.O. Box 806 Sacramento, CA 95812-0806 Immediately locate a copy of the missing manifests or prepare and submit the required information to DTSC. Submit to the EHD a copy of the signed off copy of the manifests or a copy of the information submitted to DTSC. ❑ This violation was corrected ❑This violation will be corrected by(date): ❑ Supporting documents included Describe actions taken or will be taken to correct violation: Violation#301 -Facility not maintained to minimize the release of a hazardous waste. -A sales shelf in the facility,which held containers of Comet and Ajax bleach-containing cleaning powders,was found with spillage of the bleach-containing powders (IMG 5950-4). The hand-held scanner indicated the Comet and Ajax bleach-containing powders should be placed in an STOU HW tote,while the"Products To Be Treated As Hazardous Waste"guide posted by the HW storage area states"Ajax or Comet Type Cleaner(with bleach)"should be placed in an OX HW tote(IMG 5928). -A sales shelf in the facility,which held containers of Ty-D-Bol toilet cleaner,was found with dried up spillage of the Ty-D-Bol cleaner(IMG 5955-7).The"Products To Be Treated As Hazardous Waste"guide posted by the HW storage area states"Toilet Bowl Cleaner(without bleach-liquid)"should be placed in a CA HW tote(IMG 5928). Facilities shall be maintained and operated to minimize the possibility of a fire, explosion,or release of hazardous waste to air, soil, or surface water which could threaten human health or the environment. Immediately clean up the spillage and staining from the Comet and Ajax cleaning powders and the Ty-D-Bol toilet cleaner, and manage according to Title 22 hazardous waste regulations.
