OBSERVATION: The designated operator failed to document all the alarms from the attached alarm history on the 01/08/2020 designated operator inspection report and failed to check that they were responded to appropriately. The missing alarms include:L7 , 87 annular space,sensor out alarm on 01/3/2020. The designated operator failed to check and note all required items on the visual inspection reports. The incorrect and missing information included the following from the inspection reports: -The DO report of 09/17/2020 did not include the owner/operator name. The designated operator inspections for the following months were performed more than 30 days after the previous inspection: - Initial inspection on 05/27/2020, next inspection performed on 08/19/2020 (84 DAYS BETWEEN THE 2 DATES) REGULATION GUIDANCE: The designated operator visual inspections shall be conducted within 30 days or less of the previous inspection. CORRECTIVE ACTION: Ensure that all designated operator inspections are performed within the required time frames. This is a minor violation. 210 CCR 2715(c) Failure to provide initial training, train every 12 months, and have a trained employee present. OBSERVATION: The designated operator employee training for Arlene Gayeta was not performed. REGULATION GUIDANCE: The designated operator shall train facility employees in the proper operation and maintenance of the UST system at least once every 12 months. The training shall be conducted before the individual performs the duties of a facility employees. The training shall include, but is not limited to: 1. Operation of the UST system in a manner consistent with the facility's best management practices 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan 3. Employee's role with regard to spills and overfills as specified in the facility's response plan 4. Name of the contact person(s)for emergencies and monitoring equipment alarms Initial training shall also include a site-specific practical demonstration that provides both sharing information and evaluating knowledge for responding to emergencies, leak detection, monitoring alarms, spills and overfills. At least one of the facility employees present during operating hours shall have current training. CORRECTIVE ACTION: Ensure that employees have been trained by the designated operator, maintain the list on site, and submit a copy of the training records to the EHD. This is a Class II violation.
