The incorrect <br /> and missing information included the following from the inspection reports: <br /> -The DO report of 09/17/2020 did not include the owner/operator name. <br /> The designated operator inspections for the following months were performed more than 30 days after the previous <br /> inspection: <br /> - Initial inspection on 05/27/2020, next inspection performed on 08/19/2020 (84 DAYS BETWEEN THE 2 DATES) <br /> REGULATION GUIDANCE: The designated operator visual inspections shall be conducted within 30 days or less of <br /> the previous inspection. <br /> CORRECTIVE ACTION: Ensure that all designated operator inspections are performed within the required time <br /> frames. <br /> This is a minor violation. <br /> 210 CCR 2715(c) Failure to provide initial training, train every 12 months, and have a trained employee present. <br /> OBSERVATION: The designated operator employee training for Arlene Gayeta was not performed. <br /> REGULATION GUIDANCE: The designated operator shall train facility employees in the proper operation and <br /> maintenance of the UST system at least once every 12 months. The training shall be conducted before the <br /> individual performs the duties of a facility employees. The training shall include, but is not limited to: <br /> 1. Operation of the UST system in a manner consistent with the facility's best management practices <br /> 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br /> 3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br /> 4. Name of the contact person(s)for emergencies and monitoring equipment alarms <br /> Initial training shall also include a site-specific practical demonstration that provides both sharing information and <br /> evaluating knowledge for responding to emergencies, leak detection, monitoring alarms, spills and overfills.
