March 26, 1993 1 removalrigoteme To All Our Valued Vendors: Effective April 1, 1993, we will be conducting all business under the name California Waste Removal Systems, Inc. The business names of Sanitary City Disposal, SANCO Disposal, and Independent Disposal Service will no longer be used, although ownership and management remains the same. Please update your files to reflect this name change. In addition to this name change, we have also updated our Purchase Order System. Please make note of the following procedures if you currently receive purchase orders from us: 1 . All invoices must have a purchase order number on them. 2. We will be unable to pay any invoice without a purchase order number. 3. All invoices must be priced in detail at the time of purchase. 4. All invoices must be itemized in detail. For example: all tax, freight, core charges, or special handling fees must be listed, included, and be reflected in the total amount due. Note: Any freight charges that are taxed will not be paid. 5. Purchases in quantity must have the prices extended. 6. Back orders require a separate purchase order number and it must be issued prior to shipment. 7. All purchase orders require a signature from authorized personnel of California Waste Removal Systems, Inc. 8. Only purchase orders with the following authorized signatures can be accepted by vendors: David Vaccarezza, Annette V. Murdaca, Thomas Sanchez, Dale E. Thompson, Jerry Mayberry, and Kevin Walker. 9. Our Federal I.D. Number for California Waste Removal Systems, Inc., is 94-2179069. We feel this name change will increase efficiency and assist in simplifying our accounting procedures.
