Box 241001 <br /> Lodi, CA 95241 <br /> Dear Mr. Fiori : <br /> SUBJECT: California Waste Removal Systems (39-TI-0078) <br /> The California Integrated Waste Management Board (CIWMB) staff <br /> has reviewed Form CIWMB 500 submitted by the operator for the <br /> above-named site located at 1333 East Turner Road, Lodi, <br /> California. <br /> Based on information provided on Form CIWMB 500, it has been <br /> determined that the operator of the waste tire site named above <br /> is not required to obtain a minor or major waste tire facility <br /> permit . Exhibit A of this letter and the documents referenced in <br /> Exhibit A, if any, describe the operation and the factual <br /> background that qualifies the operator for exclusion from waste <br /> tire facility permit requirements . Exhibit A and any referenced <br /> documents are attached. This site remains subject to inspection <br /> by the CIWMB. <br /> This letter does not release the operator/owner from his/her <br /> responsibilities under any other existing laws, ordinances, <br /> regulations, or statutes of other government agencies. <br /> If the conditions described in Exhibit A change or the statutes <br /> or regulations are revised, you may have to get a waste tire <br /> facility permit from the CIWMB. The operator/owner must inform <br /> the CIWMB of any change in the operation, operator, or owner. <br /> Any change in the operation, operator, or owner brought to the <br /> attention of the CIWMB will be reviewed to determine if the <br /> operator still qualifies for exclusion from waste tire facility <br /> permitting requirements. The CIWIviB will determine if a new <br /> application and/or a site inspection are necessary. <br /> This letter, with attachments, may be shown to haulers as proof <br /> that the operator of the above-named site may legally accept <br /> waste tires without a minor or major waste tire facility permit . <br /> I encourage you to call the CIWMB Tire Unit staff to answer any <br /> questions on waste tire facility permit issues .
