Gear Oil—the hazardous materials type element has not been completed. <br /> 2. Propane—the days on site element has not been completed. <br /> 3. Acetylene—the hazardous materials type element has not been completed and the storage <br /> temperature is wrongly identified as"above ambient"vs. "ambient. <br /> 4. Oxygen -the storage temperature is wrongly identified as "above ambient"vs. "ambient. <br /> S. Hydraulic oil—the hazardous materials type element has not been completed. <br /> 6. Waste antifreeze—the annual waste amount element has not been completed. <br /> 7. Waste oil—the annual waste amount element has not been completed. <br /> • Violation #6,the site map does not show or address the following(see attached map guidance doucments): <br /> 1. A map showing the entire facility with adjacent streets. <br /> 2. Map 1 and map 2 are missing. <br /> 3. One map was submitted without being numbered. <br /> 4. Loading areas. <br /> S. Internal roads. <br /> 6. Storm and sewer drains. <br /> 7. Access and exit point for the structures and from the adjacent streets. <br /> 8. A legend to identify each of the codes or symbols used on the map was not provided. <br /> A site map shall contain a north orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, <br /> access and exit points, emergency shutoffs, evacuation staging areas, hazardous material handling and storage areas, <br /> and emergency response equipment. If a site map element is not applicable for your facility then list it on the map and <br /> label as "NA". <br /> • Violation #7, Emergency Response/Contingency Plan is not complete. <br /> 1. Section C. the phone numbers for the State Warning Center and the Local Unified Program Agency were <br /> not included in the uploaded documents. <br /> 2. Documents not relevant to an Emergency Response/Contingency Plan were uploaded. <br /> 1 <br />
