The EHS box was marked yes in <br />the chemical inventory section for three chemicals with triangles. Please make sure all chemicals say no for EHS. <br /> Any material that meets or exceeds the reportable quantity shall be reported in the facility's business plan. <br />This violation was corrected on site. <br />This is a minor violation. <br /> 6 HSC 25505(a)(2) Failed to electronically submit a site map with all required content. <br />The site map was not completed as part of the business plan. The site map is missing storm drains, internal roads, <br />adjacent streets, emergency response equipment (fire extinguisher, safety shower, eye was, spill kits, absorbent), <br />an evacuation staging area, Emergency shut offs for the chemical tanks, and utilities shut offs (water, natural gas, <br />and power). Please add the missing elements to the site map. <br /> A site map shall contain a north orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, <br />access and exit points, emergency shutoffs, evacuation staging areas, hazardous material handling and storage <br />areas, and emergency response equipment. If a site map element is not applicable for your facility then list it on the <br />map and label as “NA". Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br />This is a minor violation. <br /> 7 HSC 25505(a)(3) Failed to electronically submit an adequate ER procedures for a release or threatened release. <br />The emergency response(ER) procedures were not complete. The local Unified Program Agency (UPA) phone <br />number was not provided.
