111 Source and cause of the release <br />Ill A description of the affected medium (air, water, soil, etc.) <br />Ill Extent of actual and potential water pollution <br />Ill Any damages or injuries caused by the spill <br />Ill Steps being taken or proposed to contain and clean up the spill <br />Ill Steps being taken to minimize impacts <br />1111 Whether evacuation is/was necessary <br />1111 Names of individuals/organizations who have also been contacted <br />• A written report will be submitted to the U.S. Environmental Protection Agency within <br />30 days, California Office of Emergency Services and the Central Valley Regional <br />Water Quality Control Board within 60 days if the spill exceeds 1,000 gallons or <br />occurs within a year of a previous reportable spill.
