HEALTH AND SAFETY EVALUATION-FORM 3 <br /> Hazard Assessment <br /> Background Review: ® Complete ❑ Partial If partial why? <br /> Activities Covered Under This Plan: <br /> No. Task/Subtask I Description Schedule <br /> I Soil sampling One 4-point composite surface soil sample will be collected from each of the 12 March 2019 <br /> DUs across the Site and will be analyzed for lead.One 4-point composite shallow (tentative) <br /> subsurface(approximately 2 ft bgs)soil sample will be collected from each of the <br /> 12 DUs across the Site(to be colocated with the surface soil samples)and will be <br /> analyzed for lead.Three soil borings will be advanced across the site to a total depth <br /> of approximately 20 ft bgs using direct-push methodology.Soil will be screened for <br /> organic vapors using a photo-ionization detector(PID)at 4-ft intervals.Soil samples <br /> with elevated organic vapors(as measured with the PID)and/or samples collected <br /> from approximately 10 and 20 ft bgs will be analyzed for lead,TPH-g,TPH-d,TPH- <br /> mo,PAHs,and VOCs. <br /> 2 Soil Gas Sampling. Soil vapor borings will be installed using direct push methodology at five, March 2019 <br /> approximately evenly-spaced locations across the Site to a total depth of (tentative) <br /> approximately 20 ft bgs.Soil will be screened for organic vapors using a PID at 4-ft <br /> intervals and soil samples may be submitted for analysis if elevated levels of organic <br /> vapors are encountered.At each boring,soil vapor probes will be installed at two <br /> selected depths(dependent on lithology)targeted for 5-7 ft bgs and 10-15 ft bgs. <br /> Soil vapor samples will be analyzed by VOCs/TPH-g via EPA Method TO-15 and <br /> for TPH-d via EPA Method TO-17 orequivalent). <br /> Types of Hazards: <br /> The number 1 refers to one of the following hazard evaluation forms.
