(ECM) is submitting the enclosed boring permit application <br />for the Supplemental ISCO Injection Project at the California Army National Guard Field <br />Maintenance Shop #24 located in Stockton, California. ECM has included the following as part <br />of the application package: <br />• Completed San Joaquin County — Environmental Health Department (SJEHD) Boring <br />Permit Application, <br />• Completed Master File Record Information (MFR) form, <br />• Brief work scope summary, <br />• Boring plan figure, and <br />• Check for $520 to cover the Boring Permit Fee ($130) and three separate Inspection Fees <br />($390). <br />We will be completing a total of 29 borings in three (3) mobilizations. The first mobilization is <br />scheduled during the week of June 30, 2015 and will entail drilling through the concrete slab 10 <br />to 12 feet bgs using hollow stem and then a star bit or air hammer will be used to break through <br />the slab. Once the concrete has been passed the boring will be temporarily terminated and sealed <br />using bentonite and sand until the ISCO injections. The second mobilization will commence the <br />supplement ISCO injections during the week of July 6, 2015 with the last ISCO injection <br />mobilization under this permit during the week of August 10, 2015.
