Hwy 88 Lockeford, CA 95237 SR0064416 <br /> The San Joaquin County Environmental Health Department (EHD) has completed the review of the <br /> application for a permit to install a new underground storage tank system at the above noted address. <br /> The permit has been approved with the following conditions: <br /> 1. The San Joaquin Valley Air Pollution Control District for applicable permits. <br /> 2. If contamination is found in the UST pit, then the new UST install cannot be done in the <br /> contaminated UST pit until the SJC EHD Unit 4 contamination remediation requirements have <br /> been met. A work plan must be submitted to the SJC EHD Unit 4. Removal of contaminated soil <br /> must be supervised by Unit 4. <br /> 3. Qualified installers must possess or work under the direct and personal supervision of an <br /> individual physically present at the work site, at all times while work is being performed, who <br /> possesses all of the following: <br /> • One of several permissible valid, current contractor's licenses issued by the <br /> Contractors State License Board (may be issued to the installation company), and <br /> • A valid, current UST Installation/Retrofitting certificate issued by the International Code <br /> Council (ICC), and <br /> • A valid, current certificate of training from the manufacturer(s) of the component(s) <br /> being installed. <br /> Additionally, <br /> Each person installing or testing any component must hold a certificate of training from the <br /> manufacturer of that component. <br /> 4. Qualified service technicians must possessor work under the direct and personal supervision of <br /> an individual physically present at the work site, at all times while work is being performed, who <br /> possesses all of the following: <br /> • One of several permissible valid, current contractor licenses issued by the Contractors <br /> State License Board (may be issued to the installation company), and <br /> • A valid, current California UST Service Technician certificate issued by the <br /> International Code Council (ICC), and <br /> • A valid, current certificate of training from the manufacturer(s) of the component(s) <br /> being tested, repaired, or serviced. <br /> Additionally, <br /> Page ] of 3 <br />
