Hwy 88 Lockeford, CA 95237 SR0064417 <br /> The San Joaquin County Environmental Health Department (EHD) has completed the review of <br /> the application for a permit to remove the following underground storage tank(UST)systems, <br /> including piping and associated appurtenances, at the above noted address: <br /> Tank#39-0162205: 10,000 gallon regular unleaded <br /> Tank#39-0162206: 10,000 gallon midgrade unleaded <br /> Tank#39-0162207. 10,000 gallon premium unleaded <br /> The permit has been approved with the following conditions: <br /> 1. Contact the San Joaquin Valley Air Pollution Control District for any applicable permits <br /> and requirements. <br /> 2. Contact the Mokelumne Fire District at least 48 hours prior to the tank removal. <br /> 3. Any changes in information and scope of work must be submitted to EHD for approval <br /> prior to commencing work. All new addenda supersede the previous ones. <br /> 4. Contact Aris Cacapit at(209)468-3453 to arrange the following inspection appointments <br /> (a 48-hour notice is required for inspection appointments). <br /> a. Removal of tanks and piping <br /> b. Witness soil and/or water sampling <br /> c. Verification of Tank ID numbers marked on the USTS <br /> 5. If the removed backfill material and soil is found to be contaminated it must be disposed <br /> of at an approved disposal location. Treatment of soil onsite can only be performed <br /> under a hazardous waste treatment permit. A work plan and fees must be submitted to <br /> the EHD Site Mitigation Unit and the work plan must be approved before initiating any <br /> assessment or remediation activities. <br /> 6. A copy of the submitted"Health and Safety Plan`must be on the jobsite for review. <br />
