All piping, fittings, sumps, under-dispenser containment and tanks must be certified by an <br /> independent testing organization and labeled as required by the organization, (UL, ULC, ANSI, <br /> API, ASTM, etc. See Title 23 Chapter 16 Table B for complete list.) Pursuant to Underwriters <br /> Laboratories Inc (UL) listing requirements, only those products bearing the UL mark will be <br /> considered listed. ANY PART/PRODUCT NOT MARKED WITH THE UL LABEL OR ANOTHER <br /> LISTED TESTING ORGANIZATION WILL BE REJECTED. <br /> 6. All USTs shall be anchored using deadmen as detailed in the permit application, <br /> 7. All monitoring systems shall be listed in the LG 113 letter. <br /> 8. All installation and testing shall be in compliance with manufacturer's instructions, section 2635 <br /> and section 2636 of California Code of Regulations (CCR). <br /> 9. All piping must slope to a containment sump with an audible/visual alarm. <br /> 10. All tank top openings that are connected to the primary space of the UST must have secondary <br /> containment, audible/visual alarm and be vapor tight or terminate at grade. <br /> 11. Sumps and under dispenser containment must be constructed in a manner to prevent vapors from <br /> entering the backfill. <br /> 12.All secondary containment must be constructed to prevent water intrusion. <br /> 13 The secondary containment system must be continuously monitored as required under California <br /> Health & Safety Code (HSC) 25290.1 <br /> 14. Overfill prevention shall satisfy CCR 2635(b)(2)(C) pursuant to the permit application. <br /> 15.All metal components in direct contact with the ground such as flexible connectors, swing joints, <br /> fittings,clamps, bands, and pumps shall have corrosion protection. <br /> 16.Any changes shall be submitted to the EHD for approval prior to commencing work. All new <br /> submittals supersede the previous ones. <br /> 17.A "Certification of Installation /Modification"form for each tank shall be submitted by the owner or <br /> the owners'agent[pursuant to California Code of Regulation (CCR) 2635(d)] before this install will <br /> be considered complete by EHD. <br /> 18.An accurate and drawn to scale as-built plan must be submitted to the EHD prior to scheduling a <br /> final inspection. <br /> 19. No gasoline, diesel, or other hazardous material or waste can be released (intentionally or <br /> unintentionally) to the sewer/septic, storm water system, or any other waters of the state. if a <br /> release occurs it will be considered illegal hazardous waste disposal and will be subject to <br /> enforcement. <br /> 20. Submit the Best Management Practices (BMP) to EHD for managing fuel spills and wash-down <br /> water at this site prior to the final inspection. Include in your BMPs how fuel or any other liquid <br /> will be removed from the vapor spill buckets as they contain no draining mechanism. <br /> 21. Submit correct and complete Monitoring and Response Plans prior to scheduling a final <br /> inspection. <br /> 22. Submit a Certification of Financial Responsibility for Underground Storage Tanks containing <br /> petroleum form with the appropriate mechanism prior to scheduling a final inspection. <br /> 23. Passing Enhanced Leak Detection(ELD) results must be received prior to receiving fuel. <br /> 24, Contact Aris Cacapit, REHS at (209)468-3453 for inspections. See the inspection checklist for the <br /> required Inspections. All appointments must be scheduled 48 hours in advance. <br /> 25. This permit expires at the end of the calendar year.
