S A N J 0 A Q I I I Irl Environmental Health Department <br /> CCUN r <br /> Underground Storage Tank Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> STOP N SHOP 1 1856 W COUNTRY CLUB BLVD, STOCKTON August 31, 2020 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 210 CCR 2715(c)No employee present that were trained in the operation and maintenance of the UST system by a DO. <br /> OBSERVATION: The designated operator employee training for Abraham Muhammad was not performed and he <br /> was the only employee on site at the start of the inspection. <br /> REGULATION GUIDANCE: The designated operator shall train facility employees in the proper operation and <br /> maintenance of the UST system at least once every 12 months. The training shall be conducted before the <br /> individual performs the duties of a facility employees.The training shall include, but is not limited to: <br /> 1. Operation of the UST system in a manner consistent with the facility's best management practices <br /> 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br /> 3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br /> 4. Name of the contact person(s)for emergencies and monitoring equipment alarms <br /> Initial training shall also include a site-specific practical demonstration that provides both sharing information and <br /> evaluating knowledge for responding to emergencies, leak detection, monitoring alarms, spills and overfills. At least <br /> one of the facility employees present during operating hours shall have current training. <br /> CORRECTIVE ACTION: Ensure that employees have been trained by the designated operator, maintain the list on <br /> site, and submit a copy of the training records to the EHD. <br /> This is a Class II violation. <br /> 214 CCR 2712 Failure to comply with UST operating permit conditions. <br /> OBSERVATION: Per Ahmed Muthanna, he has been the new tank operator for approximately 11 months. A permit <br /> transfer has not been completed with the EHD and all required elements have not been submitted to CERS within <br /> 30 days of the tank operator change. <br /> REGULATION GUIDANCE: Permits may be transferred to new underground storage tank owners if: <br /> 1)the new UST owner does not change conditions of the permit; and <br /> 2)the transfer is reported to the local agency by submitting all of the required submittal elements in CERS within 30 <br /> days of the change in ownership <br /> CORRECTIVE ACTION: Immediately take all necessary actions to bring these UST systems into compliance. <br /> This is a Class II violation. <br /> 319 CCR 2635, 2637.2,2665, 2712 Failure to comply with the installation, inspection, or record keeping of the overfill <br /> prevention. <br /> OBSERVATION: Overfill prevention testing has not been conducted. <br /> REGULATION GUIDANCE: Overfill prevention equipment inspections were required to be conducted by October <br /> 13, 2018, and every 36 months thereafter, and within 30 days of completion of a repair. <br /> CORRECTIVE ACTION: Immediately schedule this testing and provide 48 hours notification to the EHD.
