SANJOAQUIN <br />COUNTY— <br />Environmental Health Department <br />Underground Storage Tank Program Inspection Report <br />Facility Name: <br />Facility Address: <br />Date: <br />DBA CIRCLEK, REFUEL PETROLEUM INC. <br />419 S MAIN ST, MANTECA <br />May 06, 2021 <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />Item # <br />Remarks <br />203 <br />CCR 2712(b)(1-2) Failure to maintain monitoring records for 36 months and maintenance records for 60 months. <br />OBSERVATION: Maintenance and monitoring records for the last three years were not found on site. <br />- The 2020, 2019 and 2018 monitoring system certification test results were not found on site during the inspection. <br />- The 05/08/2019 overfill prevention test resuts were not found on site during the inspection. <br />- The 05/15/2018 seconsdary containment test resuts were not found on site during the inspection. <br />REGULATION GUIDANCE: These records shall be maintained on site for at least three years. Monitoring records <br />include: (1) date and time of all monitoring or sampling; (2) monitoring equipment calibration and maintenance <br />records; (3) results of any visual observations; (4) results of sample analysis performed a lab or in the field; (5) logs <br />of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6) results of <br />inventory readings and reconciliations. <br />CORRECTIVE ACTION: Immediately locate and maintain all missing maintenance and monitoring records for the <br />last three years on site and submit copies to the EHD. <br />This is a repeat violation, Class ll. <br />206 <br />CCR 2712(i), 2634(e) Failure to have a current UST Response Plan available on site. <br />OBSERVATION: An approved copy of the response plan was not available for inspection. <br />REGULATION GUIDANCE: A copy of this plan shall be accessible on site at all times. <br />CORRECTIVE ACTION: Immediately locate and retain a copy on site. <br />This is a minor violation. <br />318 <br />CCR 2635, 2637.1, 2665, 2712 Failed to meet spill container requirements. <br />OBSERVATION: The 91 direct -bury spill container failed when tested. <br />REGULATION GUIDANCE: All spill containers shall have a minimum capacity of five gallons and be capable of <br />containing a spill or overfill until it is detected or cleaned up. <br />CORRECTIVE ACTION: Immediately discontinue deposition of petroleum into this tank until the component is <br />repaired or replaced by a properly licensed, trained, and certified contractor under permit and inspection of the EHD. <br />If the spill container can't be replaced immediately, there is a possibility that the 91 UST system may be red tagged <br />to prevent fuel inputs. <br />This is a repeat violation, Class ll. <br />Overall Inspection Comments: <br />Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br />documenting the corrective actions that have been or will be taken for each violation, and any supporting <br />paperwork, by 06/05/2021 . <br />FA0003685 PR0231433 SCO01 05/06/2021 <br />EHD 23-01 Rev. 9/16/2020 Page 5 of 6 Underground Storage Tank Program OIR <br />1868 E.
